Short Description
The initiative allows culturally competent and linguistically appropriate services to be offered where, and when clients need them, and successful applicants must have the ability to make immediate adjustments to the outreach plan based on input from clients, disease outbreaks, and special community events. The aim of this funding is to reduce health disparities and health inequities in preventative health screenings in medically underserved communities and disadvantaged and vulnerable communities of color in Illinois. All services in conjunction with this grant must have established partnerships in place to provide linkage to care and other wrap-around services as needed. The Initiative is funded through State of Illinois general revenue funding and will provide a variety of preventative screenings. Screenings must include, but are not limited to, HIV, Hepatitis C, and other sexually transmitted infections. Health equity exists when all people have the opportunity to thrive and no one is limited in achieving comprehensive health and wellness because of their social position or any other social factors/determinants of health such as income, education, race/ethnicity, sexual identity, and disability.

Applicant Eligibility
Regardless of the source of funding (federal pass-through or State), all grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active DUNS number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State. Applications must be submitted via the Illinois Department of Public Health's Electronic Grants Administration and Management System (EGrAMS), accessible at idphgrants.com.
Criteria Selecting Proposals
Upon submission, applications will be reviewed for completeness and for consistency with program parameters. Applications will then be scored out of 100 points scored as follows: Needs 10% Capacity 10% Clearly Defined Scope of Work 10% Quality 10% SMART 10% Check List Compliance 35% Quality 15%
Award Procedures
Applicants will receive notification via Egrams of the award once all of the applications in E-grams are reviewed and scored.

Appeals
For merit based reviews, only the evaluation process is subject to appeal. Competitive grant appeals are limited to the evaluation process. Evaluation scores or award decisions may not be protested.
Renewals
The WOW grant is a three -year grant competitively awarded in year 1 with the option of 2 years non-competitive renewal applications. Funding for subsequent years shall be contingent upon available funds, and applicant performance evaluation conducted by the Illinois Department of Public Health.

Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Records Retention. Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the records must be retained until all litigation, claims or audit exceptions involving the records have been resolved and final action taken.
