Short Description
Special Children's Charities (SCC) funds the Special Olympics Chicago (SOC) programs and other athletic programs, which are administered by the Chicago Park District (CPD) and the Chicago Public Schools (CPS). Special Children’s Charities program played a key role education, social and physical development of individual with development disabilities. Unfortunately, due to COVID-19, all in-person programs, including sports competitions, social opportunities, and seasonal camps were cancelled leaving a huge void for their athletes. Before COVID-19, SCC programs reached over 7,500 athletes and 150 Chicago Public Schools and today the participation levels are approximately 20% of the pre-pandemic level and dropped to 41 schools. The program population is 47.93% Latinx, 36.98% African American, 9.19% White, and 5.91% other. 83% of students are “Economically Disadvantage,” coming from families whose income is within 185% of the federal poverty line. During the pandemic, low-income families had fewer resources to help address the problems created by COVID-19. Many lower income workers did not have the ability to work from home where they could have been assisted with remote learning or other virtual activities through SCC/SOC. As a result, many students with developmental disabilities experienced major learning and skill loss as result of remote learning and loss of additional support services like special education aides and modified curriculum. Athletes relied on specialized services from trained professionals specific to their disabilities to provide not only learning options, but also athletic, recreational, and social outlets. As a result of COVID-19 closures, SCC participants faced extreme isolation creating social and emotional problems. To mitigate those impacts, SCC seeks to offer virtual learning programs, After 22 for adults with I/DD, Science, Art and Music programs to address the educational, social, and mental health needs of their students. The use of ARPA funds will help us address the terrible impact that COVID, and specifically the transition to remote learning has had on Chicago students with developmental disabilities. Deliverables: 1. The grantee will provide a list of unduplicated individuals participated in the After 22 Program. a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, county in which they reside and school the individual attends. b. For each participant, provide the name of “opportunity liaison” who worked with the participant to identify academic, professional, and social goals, and brief description of goals set. c. Provide the name of the academic and occupational course work that was provided to the participant and the date of when these courses are offered. 2. The grantee will provide a list of unduplicated individuals participants in the Museum of Science and Industry Science and Technology Program. a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, and county in which they reside, school the individual attends, provide name of virtual class and/or family event, date attended. b. Provide a list of all virtual classes and family event offer for the quarter, date, location of event and number of unduplicated participants in attendance. 3. The grantee will provide a list of unduplicated individuals that participated in the Art and Music Therapy Program. a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, county in which they reside and school the individual attends. b. Provide dates of group art and music therapy sessions, location, and number of unduplicated participants in attendance. 4. The grantee will provide a list of unduplicated individuals participated in the Cooking and Comedy classes. a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, county in which they reside, and the school they attend. b. Provide dates of cooking and/or comedy classes, topic of discussion, location, and total number of unduplicated participants in attendance. 5. The grantee will provide a list of unduplicated individuals participated in the Intramural team league and Skills Clinics. a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, county in which they reside and indicate which program(s) the individual participated in. b. Provide dates of Intramural team league events and skills clinics, location, and number of unduplicated participants in attendance. 6. For each program, the grantee will provide a copy of the agency's survey and a summary of the responses, quarterly, from the Satisfaction Survey. Surveys should be distributed to the client or the client’s representative after service has been provided, based on the program. (Example; participant in an ongoing class, would have 1 survey per quarter). a. Required data: Provide a blank copy of survey. b. Provide the number of surveys distributed (each client served should have been provided with a survey). c. Provide a summary of responses to each question (this can be one survey with totals notated on each question).

Objective
Special Children’s Charity intends to: • Address the educational disparities that exist by providing services to more Chicagoans with developmental disabilities, specifically targeting CPS students from low-income families. • Re-establish the participation of athletes that were involved in our event’s pre-pandemic. • Provide life-enriching programs and services that address the educational, social, emotional, and mental health needs of students. • Provide assistance to Chicago Public Schools, a high poverty school district serving a population that is over 83% low-income.

Reports
Reporting Requirements: A. Time Period for Required Periodic Financial Reports. Unless a different reporting requirement is specified in Exhibit E, Grantee shall submit financial reports to Grantor pursuant to Paragraph 10.1 and reports must be submitted no later than 30 days after the quarter ends. B. Time Period for Close-out Reports. Grantee shall submit a Close-out Report pursuant to Paragraph 10.2 and no later than 30 days after this Agreement's end of the period of performance or termination. C. Time Period for Required Periodic Performance Reports. Unless a different reporting requirement is specified in Exhibit E, Grantee shall submit Performance Reports to Grantor pursuant to Paragraph 11.1 and such reports must be submitted no later than 30 days after the quarter ends. D. Time Period for Close-out Performance Reports. Grantee agrees to submit a Close-out Performance Report, pursuant to Paragraph 11.2 and no later than 30 days after this Agreement's end of the period of performance or termination. The Providers will submit report to the Division of Developmental Disabilities (DDD) on a quarterly basis, utilizing the Periodic Performance Report (GOMBGATU-4001). Quarterly reports will be submitted no later than 15 days after end of each report period. 1st Quarter Reports are due No Later Than (NLT) October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th. At the end of the year, successful applicants will also submit a cumulative report with the cumulative totals from above. These reports are to be email to DHS.DDDBCR@illinois.gov with subject with the subject line stating: PPR, the Reporting Quarter, and Provider Organization Name. All PPRs should be returned in the pdf format, with signature. Grantees may be required to submit supporting documentation for their requests at the request of and in a manner prescribed by the Grantor. Failure to comply with these reporting requirements could result in the Department placing you on the stop pay list, withholding of funds, termination of the grant agreement and subject to the Grant Funds Recovery Act.
Audits
In accordance with the provisions of 2 CFR 200, Subpart F - Audit Requirements, non-Federal entities that expend financial assistance of $750,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Non-Federal entities that expend less than $750,000 a year in Federal awards is exempt from Federal audit requirements for that year, except as noted in 2 CFR 200.503.
