Short Description
Special Olympic Illinois ARPA funds will be used to recover from the negative economic impact resulting from the COVID-19 pandemic – being loss of revenue, athletes, volunteers, school engagement, Torch Run Officers, partnerships and overall community engagement. These funds will allow them to get back to serving the community at the level held prior to the pandemic. This funding will allow us, once again, to accomplish our mission to provide year-round sports training and athletic competition in a variety of Olympic-type sports for children and adults with intellectual disabilities, giving them continuing opportunities to develop physical fitness, demonstrate courage, experience joy and participate in a sharing of gifts, skills and friendship with their families, and other Special Olympics athletes and the community. Due to the Covid-19 pandemic, Special Olympics Illinois ceased programming in March 2020. For many of the Athletes, Special Olympics Illinois is the only socialization and physical activity that they experience. They meet their friends and feel a like a member of a community. As of today, many of these athletes still have not returned to the program and some may not ever. Special Olympic has created innovative ways to bring their program to them and provide services to the agencies/facilities that house our athletes. This funding is essential to provide the program to individuals who may be isolated or living in low-income communities. To mitigate the negative impact of COVID-19, Special Olympics seeks to offer a various of programs to its Athletes. These programs are Athlete Leadership, Healthy Athlete Program, Motor Activities Training Program, eSports, Unified Champion Schools (transformative education program) and Law Enforcement Torch Run. Deliverables: 1. The grantee will provide a list of unduplicated individuals participated in the Athlete Leadership Program. a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, and county in which they reside. b. For each participant, provide the name of activities or event and date attended. 2. The grantee will provide a list of unduplicated individuals participants in the Healthy Athlete Program. a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, and county in which they reside. b. For each participant, provide the type of health screening/ exam or education received and date of service. 3. The grantee will provide a list of unduplicated individuals that participated in the Motor Activities Training Program. a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age and county in which they reside. b. For each participant, provide the type of MAT received and date of training. 4. The grantee will provide a list of unduplicated individuals participated in the eSports Program. a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age and county in which they reside. b. For each participant, name and date of event attended. 5. The grantee will provide a list of unduplicated individuals participated in the Unified Champion School (transformative education program). a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age and county in which they reside. b. For each participant, name and date of event attended. 6. The grantee will provide a list of unduplicated individuals participated in the Law Enforcement Torch Run. a. Data Required: Provide the individual first and last name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age and county in which they reside and date of Torch Run event. 7. For each program, the grantee will provide a copy of the agency's survey and a summary of the responses, quarterly, from the Satisfaction Survey. Surveys should be distributed to the client or the client’s representative after service has been provided, based on the program. (Example; participant in an ongoing class, would have 1 survey per quarter). a. Required data: Provide a blank copy of survey. b. Provide the number of surveys distributed (each client served should have been provided with a survey). c. Provide a summary of responses to each question (this can be one survey with totals notated on each question).

Objective
Special Olympics of Illinois intends to: • Engage children with intellectual disabilities through developmentally appropriate play activities designed to foster physical, cognitive, and social development. • Welcome family members of children with intellectual disabilities to the Special Olympics network of support. • Raise awareness about the abilities of children with disabilities through inclusive peer participation, demonstrations, and other events. • Promote positive attitudes and behavior among students without disabilities toward their peers with disabilities.

Reports
Reporting Requirements: A. Time Period for Required Periodic Financial Reports. Unless a different reporting requirement is specified in Exhibit E, Grantee shall submit financial reports to Grantor pursuant to Paragraph 10.1 and reports must be submitted no later than 30 days after the quarter ends. B. Time Period for Close-out Reports. Grantee shall submit a Close-out Report pursuant to Paragraph 10.2 and no later than 30 days after this Agreement's end of the period of performance or termination. C. Time Period for Required Periodic Performance Reports. Unless a different reporting requirement is specified in Exhibit E, Grantee shall submit Performance Reports to Grantor pursuant to Paragraph 11.1 and such reports must be submitted no later than 30 days after the quarter ends. D. Time Period for Close-out Performance Reports. Grantee agrees to submit a Close-out Performance Report, pursuant to Paragraph 11.2 and no later than 30 days after this Agreement's end of the period of performance or termination. The Providers will submit report to the Division of Developmental Disabilities (DDD) on a quarterly basis, utilizing the Periodic Performance Report (GOMBGATU-4001). Quarterly reports will be submitted no later than 15 days after end of each report period. 1st Quarter Reports are due No Later Than (NLT) October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th. At the end of the year, successful applicants will also submit a cumulative report with the cumulative totals from above. These reports are to be email to DHS.DDDBCR@illinois.gov with subject with the subject line stating: PPR, the Reporting Quarter, and Provider Organization Name. All PPRs should be returned in the pdf format, with signature. Grantees may be required to submit supporting documentation for their requests at the request of and in a manner prescribed by the Grantor. Failure to comply with these reporting requirements could result in the Department placing you on the stop pay list, withholding of funds, termination of the grant agreement and subject to the Grant Funds Recovery Act.
Audits
In accordance with the provisions of 2 CFR 200, Subpart F - Audit Requirements, non-Federal entities that expend financial assistance of $750,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Non-Federal entities that expend less than $750,000 a year in Federal awards is exempt from Federal audit requirements for that year, except as noted in 2 CFR 200.503.
