Short Description
Through the COVID Health Equity Pandemic Health Navigator Project grant, the grantee and its sub-recipients will identify stakeholders and facilitate advisory groups to build and communicate a plan to increase access and use of COVID-19 testing, vaccination, and supports during the isolation/self-quarantine; identify community strengths/ opportunities and develop and update as needed a workplan and measures for each county/community; collaborate with IDPH to establish a budget for transportation assistance and quarantine/ isolation support necessary to prevent the spread of COVID-19 and encourage the return to preventive and health care services.

Objective
This grant will expand and strengthen IDPH infrastructure to address COVID-19 health disparities and inequities and strengthen the community health workforce in populations that are at higher risk for COVID-19 to identify and build a community member local public health workforce that enhances and increases level of representation of the population of focus.

Eligible entity is a non-profit organizations who already has existing Pandemic Health Navigator capacity and the ability to provide coverage to counties throughout the state of Illinois excluding Cook.
Beneficiary Eligibility
Those impacted by COVID-19 in selected priority rural and minority underserved areas outside of Cook county.

Application Procedures
Applications must be submitted via the Illinois Department of Public Health's Electronic Grants Administration and Management System (EGrAMS), accessible at idphgrants.com. Applications will be reviewed and scored on the criteria outlined in the Criteria section by the Illinois Department of Public Health, Office of Health Promotion.Prevention Section.

Program staff will review application for content, work plan activities, budget proposal and required supplemental material.
Award Procedures
The Illinois Department of Public Health (IDPH) is exempt from utilizing the standard Notice of State Award issued on the GATA Grantee Portal. Successful applicants will receive an email notification from EGrAMS and must review the funding terms and specific conditions in the grant agreement and accept utilizing an electronically signature. Both the electronic signature in EGrAMS and a physical signature on the grant agreement must be completed by an authorized representative of the grantee organization and submitted to IDPH.

The anticipated announcement will be no later than 4 weeks after the submission of the application.

Uses and Restrictions
All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for recouping of those funds used for the prohibited purpose. Expenditures must be submitted monthly. To be reimbursable under the Department/Office of Health Promotion Grant Agreement, expenditures must meet the following general criteria: • Be necessary and reasonable for proper and efficient administration of the program and not be a general expense required to carry out the overall responsibilities of the applicant. • Be authorized or not prohibited under federal, state or local laws, or regulations. • Conform to any limitations or exclusions set forth in the applicable rules, program description, or grant agreement. • Be accorded consistent treatment through application of generally accepted accounting principles, appropriate to the circumstances. • Not be allocable to or included as a cost of any state or federally-financed program in either the current or a prior period. • Be net of all applicable credits. • Be specifically identified with the provision of a direct service or program activity. • Be an actual expenditure of funds in support of program activities, documented by check number, and/or internal ledger transfer of funds. • Not be used for research. Unallowable or prohibited uses of grant funds include, but are not limited to the following: • Political or religious purposes • Contributions or donations • Fundraising or legislative lobbying expenses • Conference registration fees • Payment of bad or non-program related debts, fines or penalties • Contribution to a contingency fund or provision for unforeseen events • Research • Incentives, including but not limited to t-shirts, bags, backpacks, hats, pencils, rulers, coloring books, stress balls, band-aid holders, mugs and cookware. • Entertainment, food, alcoholic beverages and gratuities • Membership fees, interest or financial payments, or other fines or penalties • Purchase or improvement of land or purchase, improvement or construction of a building • Equipment • Expenditures that may create conflict of interest or the perception of impropriety • Exhibit fees of any kind • Association dues • Expenses for credentialing (e.g., CHES certification) • Airfare • Out of state travel costs
Reports
Financial reporting and quarterly progress and performance measure reporting are required in addition to participation in phone, video and/or in person meetings with the CDC COVID Health Equity Team as needed.

Records
Programmatic reports and fiscal documentation should be maintained for at least five years after the end date of the award.

Program Accomplishments
The grantee and its subrecipients will identify stakeholders and facilitate advisory groups to build and communicate a plan to increase access and use of COVID-19 testing, vaccination, and supports during the isolation/self-quarantine; identify community strengths/ opportunities and develop and update as needed a workplan and measures for each county/community; collaborate with IDPH to establish a budget for transportation assistance and quarantine/ isolation support necessary to prevent the spread of COVID-19 and encourage the return to preventive and health care services. Community Health Workers will work to meet with community groups and underserved individuals and families through individual and group visits to further expand COVID-19 testing, contact tracing, promotion of supports during quarantine or isolation period, acceptance of COVID-19 vaccination; help individuals design daily care needs plan during isolation and self-quarantine period; work with COVID Health Teams and utilize COVID-19 testing; educate, encourage, and help to navigate resumption of health care screenings, prevention activities, and treatments to address disease conditions; and use no-wrong door approach to meet health care and social needs of population of focus. Pandemic Health Navigators will lead regional connections with community based organizations, federally qualified health centers and work with the IDPH COVID programs and provide follow up services such as supports during isolation and essential services to stop the spread of the virus; assist CEM and CWHs in conducting proactive outreach, follow up support, and care resource support to minority and historically marginalized communities that are not part of existing contact tracing referral systems; and work closely with CHWs on team to help navigate to food, medication refills, rent/utility assistance and health care settings when needed.
