Short Description
The Illinois Department of Public Health (IDPH) administers the School Health Program which monitors the certified school health centers operating in Illinois for compliance with Title 77, Chapter I, Subchapter i, Part 641 School-Based/Linked Health Centers. LSHCE-23 will specifically support current SHC grantees receiving less than 250K per grant year under the current SHC-22 grant and have been unable to provide necessary and similar services as offered by its peers who currently receive $250K or more per grant year. More specifically, IDPH funds should not be used to supplement funds the SHC is currently able to secure and have been historically utilizing to provide existing services. Also posted as LSHCE.

Objective
Program Objectives: • Enhance current services offered through the School Based Health Center (SBHC), e.g. increased telehealth appointments, dental services, etc. • Increase percentage of clinic users through community outreach. • Increase awareness, knowledge, competency and alignment in suicide prevention, assessment, and treatment for school and school health center personnel. • Promote optimal health outcomes in regular clinic users through new services.

Applicant Eligibility
To be eligible for the Legacy School Health Center Grant Expansion Program an applicant must: 1) Be a certified school health center under 77 Ill. Adm. Code 641; 2) Be a current award recipient of the SHC-22/23 Grant; 3) Be a current grantee with an award of less than $250,000.00 (per fiscal year) from the SHC-22/23 Grant. 4) Describe the methodology for evaluating the effectiveness of the program. 5) Submit an adequate work plan with their application that should include necessary detail to enable the agency to manage the grant agreement activities and clearly define project performance metrics. 6) Attend Office of Women’s Health & Family Services (OWHFS) Conference, SHP program meetings, and SHP quarterly calls.

Applications will be reviewed and scored by IDPH School Health Program staff for completeness and accuracy as well as the criteria identified below:
Award Procedures
Upon completion of the submission and review process, each successful applicant will receive a grant agreement to be signed by the entity’s authorized official. The grant agreement is not binding on the parties until it has been fully executed by the Illinois Department of Public Health. A Notice of State Award (NOSA) shall be issued to the finalists who have successfully completed all grant award requirements and have been selected to receive grant funding. The NOSA will specify the funding terms and specific conditions resulting from applicable pre-award risk assessments. The Illinois Department of Public Health (IDPH) is exempt from utilizing the standard NOSA issued on the GATA Grantee Portal. Successful applicants will receive an email notification from the online grant system and must review the funding terms and specific conditions in the grant agreement and accept utilizing an electronically signature. Both the electronic signature and a physical signature on the grant agreement must be completed by an authorized representative of the grantee organization and submitted to IDPH. A Notice of Denial will be sent to the applicants not receiving awards.

Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the GATA section of the IDPH website (www.dph.illinois.gov/GATA). o Appeals must be received within 14 calendar days after the date that the grant award notice was published. o Appeals must include the following information: ? The name and address of the appealing party ? Identification of the grant ? A statement of reasons for the appeal ? Renewals
There are no renewal awards for this grant program. Grantees must submit a new application for each grant cycle.
Formula Matching Requirements
Matching funds (cash/in-kind contributions) are not required by the applicant. If applicant receives cash/in-kind contributions, these need to be included in the total operating budget reported in the online grant management system and specified in the grant budget summary.
Uses and Restrictions
Only 10% of the grant may be allocated to Administrative Costs, The Grantee and any approved subgrantees or subcontractors shall not expend any School Health Center Grant Funds paid from State of Illinois Funds for promotional items unless the Department’s grant manager has given written permission to do so. Promotional items include, but are not limited to: calendars, pens, buttons, pins, magnets, gift cards, posters, and stationery. State of Illinois Grant Awards will not allow reimbursement of pre-award costs, construction, or indirect costs
Reports
Monthly Reimbursement Forms will be due on the 15th of the following month; Quarterly Progress Reports will be due on the 30th of the month following the closure of the quarter; and End of Grant Report will be due forty-five days from the last date of the grant period. All reports and monthly reimbursement forms shall be submitted through the online grants management system.
Audits
Audit requirements per JCAR Title 44 Illinois Administrative Code7000.90 or alternative audit requirements specific to program.
Records
Grantees are required to maintain grant accounting records 3 years after the date they submit the Federal Financial Report (FFR). If any litigation, claim, negotiation, audit or other action involving the award has been started before the expiration of the 3-year period, the records shall be retained until completion of the action and resolution of all issues which arise from it, or until the end of the regular 3-year period, whichever is later.
