Short Description
The Illinois Department of Public Health’s Center for Minority Health Services (CMHS) is requesting applications from organizations to partner with CMHS to design and implement a framework for the CMHS Community Empowerment Zone program. The goal of the Community Empowerment Zone program is to design a strategic and structured process that will help up to 15 communities with low socio-economic ranking and low life expectancy to develop supports that improve the overall health and well-being of the communities. These are communities that have been disproportionately impacted by COVID. Applicants must provide details on how their proposed framework will address the following criteria: 1) develop and provide technical assistance and consultation to socio-economically disadvantaged and historically marginalized communities through a multi-sector approach; 2) establish a process through which select communities receive coaching and supports towards advancing health equity; and 3) build community resilience and sustainability through a health equity and social justice lens. The Community Empowerment Zone program requires the grantee to work collaboratively with participating communities, and to provide them with the tools and supports necessary to identify areas of growth. The grantee must then assist these communities in designing sustainable techniques and effective approaches that reduce obstacles to health equity. Successful applicants must: 1) , and in providing coaching and technical assistance to support the COVID related project initiatives for communities described above. 2) Demonstrate/document process for evaluating progress on RFP awards 3) Provide quarterly reporting of all funded grantee projects to IDPH 4) Provide monthly activity reporting via EGrAMS on all funded projects 5) Have the ability to manage the RFP process for selecting participating communities; 6) oversee the funding to communities selected to participate in the program; and 7) have demonstrated knowledge and experience in building collaborative community partnerships, designing and leading learning collaboratives The federal funds for this program require that the program work be focused outside of Cook county and the City of Chicago. Applicants must show that they have capacity to work in all the following counties: Alexander, Boone, Brown, Cass, Clay, Cumberland, Douglas, DuPage, Edwards, Fayette, Franklin, Hamilton, Hancock, Hardin, Henderson, Jackson, Jasper, Jefferson, Kane, Kankakee, Kendall, Lake, Lawrence, Macon, Marion, Massac, McHenry, Moultrie, Peoria, Pike, Pope, Pulaski, Rock Island, Saline, Scott, Shelby, St. Clair, Union, Vermilion, Wayne, White, Will, Winnebago Due to the complexity of this program, applicants must have at least ten years’ experience in addressing community level development with a health equity focus.

Applicant Eligibility
Legally recognized organizational entities are eligible to apply for this funding. Applicants must show that they have significant experience of at least ten years in project management related to projects that are designed to advance health equity. Eligible applicants must have lead staff with at least 15 years’ experience in working with urban and rural communities on health equity related projects. Additionally, eligible applicants must include a diverse staff that includes persons with lived experience which reflects the minority and underserved communities that are the focus of this project. Preference will be given to entities that show they will keep their administrative costs low. Legally recognized organizational entities are eligible to apply for this funding. Applications from individual persons will not be considered. This grant will require that the lead agency distributes sub-grants to participating communities

Appeals
For merit based reviews, only the evaluation process is subject to appeal. Competitive grant appeals are limited to the evaluation process. Evaluation scores or award decisions may not be protested.

This grant is a two -year grant competitively awarded in year 1 with the option of one non-competitive renewal. Funding for subsequent years shall be contingent upon available funds, and applicant performance evaluation conducted by the Illinois Department of Public Health.

Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Records Retention. Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the records must be retained until all litigation, claims or audit exceptions involving the records have been resolved and final action taken.
