PROGRAM INFORMATION
Short Description
This funding opportunity is designed to ensure that Refugees, Immigrants, and Migrants (RIM) outside of Cook County Illinois receive equitable access to tools and interventions intended to improve health outcomes through education, outreach, and community engagement. The opportunity has been created to support efforts to address COVID-19 vaccine hesitancy and access to vaccination in RIM populations and their communities. The intended outcomes are reduction in COVID-19 related health disparities via improved and increased equitable access to knowledge, COVID-19 testing, COVID-19 vaccination, and COVID-19 therapies. Additionally, special focus on RIM individuals with underlying medical conditions that increase COVID-19 risk should be included. The Refugee Health Program at the Illinois Department of Public Health will disburse grant funding to entities whose project applications help, build, leverage, and expand capacity to support culturally and linguistically appropriate COVID-19 prevention and the dissemination of related information to RIM populations via culturally and linguistically appropriate media and community engagement. Projects should be created with the ability to mobilize partners and collaborators to advance health equity and address social determinants of health as they relate to COVID-19 health disparities among RIM populations.
Federal Authorization
The Consolidated Appropriations Act, 2021 (P.L. 116-260), which contained the Coronavirus Response and Relief Supplemental Appropriations Act, 2021 (P.L. 116-260, Section 2, Division M) provided, in part, funding for strategies to improve testing capabilities and other COVID-19 response purposes in populations at high-risk and underserved, including racial and ethnic minority groups and people living in rural communities.

Criteria Selecting Proposals
Applicants will utilize the IDPH EGRAMS database to submit their grant application. Each applicant must be in good standing with the Grant Accountability & Transparency Act Unit operating under the Governor’s Office of Management & Budget - https://omb.illinois.gov. Successful applicants will have completed the GATA Pre-Qualification information and the Fiscal and Administrative Risk Assessment (Internal Controls Questionnaire – ICQ).Applications will be selected and approved based upon submission of well thought applications that align with the goals and scope of services for the NOFO. SMART Objectives are required in the work-plans.
Award Procedures
The successful applicant will receive notification via Egrams of the award once all of the applications in E-grams are reviewed and scored. Reimbursement based on monthly reporting
Deadlines
Updates to deadlines will be posted on Egrams, if applicable. The application must be complete and submitted via Egrams by the posted deadline. Late submissions will not be considered. There will be no exceptions.

Appeals
For merit based reviews, only the evaluation process is subject to appeal. Competitive grant appeals are limited to the evaluation process. Evaluation scores or award decisions may not be protested.
Renewals
This grant is a one-year grant competitively awarded in year 1. Funding for subsequent years shall be contingent upon available funds, and applicant performance evaluation conducted by the Illinois Department of Public Health.
Uses and Restrictions
The Illinois Department of Public Health’s Center for Minority Health Services (CMHS) is requesting applications from organizations to partner with CMHS Refugee Health Program to design and implement a framework to design a strategic and structured process that will help Refugee Immigrant and Migrant communities that have been disproportionately impacted by COVID. Applicants must provide details on how their proposed framework will address the following criteria: 1) develop and provide technical assistance and consultation to socio-economically disadvantaged and historically marginalized RIM communities through a multi-sector approach; 2) establish a process through which select communities receive coaching and supports towards advancing health equity; 3)Improves COVID 19 health outcomes via access to information and resources to provide care 4)build community resilience and sustainability through a health equity and social justice lens. The grant requires the grantee to work collaboratively with participating communities, and to provide them with outreach and engagement along with the tools and supports necessary to identify areas of growth. process for selecting participating communities; 6) oversee the funding to communities selected to participate in the program; and 7) have demonstrated knowledge and experience in building collaborative community partnerships, designing, and leading learning collaboratives. The federal funds for this program require that the program work be focused outside of Cook county and the City of Chicago. Applicants must show that they have capacity to work in all the following counties: Alexander, Boone, Brown, Cass, Clay, Cumberland, Douglas, DuPage, Edwards, Fayette, Franklin, Hamilton, Hancock, Hardin, Henderson, Jackson, Jasper, Jefferson, Kane, Kankakee, Kendall, Lake, Lawrence, Macon, Marion, Massac, McHenry, Moultrie, Peoria, Pike, Pope, Pulaski, Rock Island, Saline, Scott, Shelby, St. Clair, Union, Vermilion, Wayne, White, Will, Winnebago. Due to the complexity of this program, applicants must have at least ten years’ experience in addressing community level development with a health equity focus.
Reports
At minimum, approved grantees will be required to provide monthly workplan and reimbursement reports. In addition, project narratives and other reporting are to be provided at the grantors request. Further reporting instructions will be provided upon project approval.
Audits
Grantees will be audited at least annually. Additional audits will be conducted at the discretion of the awarding agency.
Records
Records Retention. Grantee shall maintain for three (7) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the records must be retained until all litigation, claims or audit exceptions involving the records have been resolved and final action taken.

Example Projects
Mobilize partners and collaborators to advance health equity and address social determinants of health as they relate to COVID-19 health disparities among RIM communities. Develop mechanisms such as community advisory groups that include leaders representing RIM groups and community leaders and members to inform COVID-19 and future emergency response activities.
