Short Description
The LHD DIS Workforce Development Support Grant is a three year non-competitive grant for $6,300,000.00 awarded to 21 local health departments. The Illinois Department of Public Health Sexually Transmitted Infections (STI) Section received supplemental American Rescue Plan Act (ARPA) funds to the core Centers for Disease Control and Prevention (CDC) Strengthening STD Prevention and Control for Health Departments (STD PCHD) federal grant to specifically support Disease Intervention Specialist (DIS) infrastructure and workforce development in Illinois. DIS services are to support COVID-19 efforts and to assist in infectious diseases such as in STI programs on field operations. This includes activities related to counseling and partner services, contact tracing, field investigations, and training.

Objective
Agency shall deliver the following in the respective LHD jurisdiction; 1. Establish new DIS positions or support part time DIS positions to be full time.2. Provide ISS with monthly summary updates on DIS personnel for newly established DIS positions and/or part time DIS positions that were made to be full time positions.3. Provide counseling, partner services, risk assessment, partner referral, and culturally competent STI and other infectious disease prevention. Services will be provided by trained Disease Intervention Specialist (DIS) who has completed a Department approved STI counselor training.4. Respond to infectious disease incidence and prevalence trends and outbreaks as reported by ISS.5. Promote infectious disease prevention and treatment services according to CDC guidelines. 6. Monitor and report cases of infectious diseases as required by law.7. Provide ISS with grantee’s standard operating procedure document(s) for the agency’s STI field operations program by July 1, 2023.Develop and utilize targeted approaches that result in programmatic improvement, within their agency, that impact communities with high morbidity.

Applicants shall be a Local Health Department listed below: DuPage County Health Department, Peoria City/County Health Department, Jackson County Health Department, Sangamon County Department of Public Health, Champaign-Urbana Public Health District, Kankakee County Health Department, Evanston Health Department, Rock Island County Health Department, McDonough County Health Department, Jefferson County Health Department, St. Clair County Health Department, Madison County Health Department, McLean Health Department, Franklin-Williamson Bi-County Health Department, Skokie Health Department, Randolph County Health Department, Lake County Health Department, Will County Health Department, Bond County Health Department, Mason County Health Department, and Clay County Health Department. Applicants may apply for this grant but will not be eligible for a grant award until they are pre-qualified through the Grant Accountability and Transparency Act (GATA) Grantee portal, www.grants.illinois.gov. During pre-qualification, Dun and Bradstreet verifications are performed including a check of Debarred and Suspended status and good standing with the Secretary of State. The pre-qualification process also includes a financial and administrative risk assessment utilizing an Internal Controls Questionnaire. If applicable, the entity will be notified that it is ineligible for award because of the Dun and Bradstreet verification. The entity will be informed of corrective action needed to become eligible for a grant award.

Uses and Restrictions
All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for recouping of those funds used for the prohibited purpose. Expenditure reports must be submitted quarterly. To be reimbursable under the Department/Office of Health Promotion Grant Agreement, expenditures must meet the following general criteria: • Be necessary and reasonable for proper and efficient administration of the program and not be a general expense required to carry out the overall responsibilities of the applicant. • Be authorized or not prohibited under federal, state or local laws, or regulations. • Conform to any limitations or exclusions set forth in the applicable rules, program description, or grant agreement. • Be accorded consistent treatment through application of generally accepted accounting principles, appropriate to the circumstances. • Not be allocable to or included as a cost of any state or federally-financed program in either the current or a prior period. • Be specifically identified with the provision of a direct service or program activity. • Be an actual expenditure of funds in support of program activities, documented by check number, and/or internal ledger transfer of funds. Unallowable or prohibited uses of grant funds include, but are not limited to the following: • Political or religious purposes • Contributions or donations • Fundraising or legislative lobbying expenses • Payment of bad or non-program related debts, fines or penalties • Contribution to a contingency fund or provision for unforeseen events • Research • Incentives, including but not limited to t-shirts, bags, backpacks, hats, pencils, rulers, coloring books, stress balls, band-aid holders, mugs and cookware. • Entertainment, food, alcoholic beverages and gratuities • Membership fees, interest or financial payments, or other fines or penalties • Purchase or improvement of land or purchase, improvement or construction of a building • Expenditures that may create conflict of interest or the perception of impropriety • Exhibit fees of any kind • Subscription costs
Reports
The grantee is required to submit quarterly progress reports on their work plan objectives. Failure to submit required reports in a timely manner will result in delays with approval of reimbursements. The grantee will ensure quarterly reports are submitted in the format provided by the Department.
