PROGRAM INFORMATION
Short Description
The purpose and scope of services for this program are to support the development
and effectiveness of new parent home visitation in Illinois by:
1. Offering support to the IDHS-DEC Home Visiting Executive Committee
2. Providing staff support to the regional IDHS Home Visiting program clusters.
3. Planning and hosting the annual Statewide Conference on Child Abuse Prevention to
include home visitor sessions track and recognition program.
4. Sponsoring an annual IDHS Home Visiting Summit training event in conjunction with the
annual statewide conference on child abuse.

Objective
The Provider will:
A. Participate in the IDHS-DEC HV governance and planning structure by:
1. Serving on the IDHS Home Visiting Statewide Cluster.
2. Co-chairing the IDHS Home Visiting Executive Committee.
3. Co-chairing the HV Summit and Conference Planning Committee
4. Participating as a member of the Early Learning Council Health and Home Visiting Committee.
B. Provide staff support to regional IDHS Home Visiting program cluster groups by:
1. Coordinating regular cluster group meetings for the five regional IDHS Home Visiting cluster
groups.
2. Preparing and sending out meeting notices.
3. Facilitating the meetings.
4. Recording, preparing, and sending out meeting minutes for meetings.
5. Performing administrative follow up tasks as needed; and
6. Serving as liaison between the cluster groups and the state level IDHS Home Visiting
committees.
C. Sponsor a Statewide Conference on Child Abuse Prevention to include a Home Visitor track
and recognition program, as indicated below:
1. Plan, coordinate, and host an annual Statewide Child Abuse Prevention Conference.
2. Coordinate with the IDHS Home Visiting Executive Committee and include at least one
home visitor track of workshop sessions consisting of at least six breakout sessions.
3. Plan and host a home visitor recognition dinner and program immediately before or after
the conference.
D. Sponsor an annual IDHS Home Visiting Summit training event as set forth below.
1. Plan, coordinate, and host a one-day annual IDHS Home Visiting Summit on the day before
the annual Statewide Child Abuse Prevention Conference.
2. Coordinate with the Home Visiting Summit and Conference Planning Committee to plan the
summit agenda and speakers.
3. Manage all the necessary arrangements for facilities, speakers, travel, etc.
4. Serve as facilitator for the Summit.
5. Conduct evaluations of the Summit and provide feedback to the various committees.
6. Using the attendee and Summit evaluations and reports, implement a continuous quality
improvement plan for supporting continuous Summit growth and success.
E. Provide technical assistance to IDHS Home Visiting program administrators and the DHS
funded IDHS Home Visiting program sites.
1. Serve as a liaison between Healthy Families America and DHS program administrative staff.
F. Quarterly reporting in the PPR should include key accomplishments and challenges
experienced during the reporting period.
G. Use the MIECHV Tracking Sheet to record publications, conferences, and other developments,
etc.

This Notice of State Award (NOSA) is not an agreement. This NOSA is not a guarantee of an agreement. IDHS will publish its agreements in the CSA Tracking System after you return a signed copy of this NOSA to IDHS.

Registration and Pre-Qualification:
Prior to applying for any Notice of Funding Opportunity (NOFO), every applicant must first be registered and prequalified through the following steps. This must be done on or before the application's due date or their application CANNOT be accepted.
•Apply for or update their DUNS number (https://www.dandb.com/ ). This must be done yearly.
•Apply for or update their SAM registration and receive a SAM cage code. (https://www.SAM.gov) This must be done yearly.
•Be registered and in good standing with the Illinois Secretary of State. (http://www.cyberdriveillinois.com/ ) (This is not required of governmental entities and schools.)
•Register with the GATA/CSFA system at https://grants.illinois.gov/registration/ . The system will automatically check your registrations with the above mentioned systems. It will also check to see if your entity is on the federal excluded parties list and if your entity is on the State of Illinois Stop Pay/Compliance list. If your entity is not up-to-date on the registrations, or is listed on the federal list or Illinois Stop Pay/Compliance list, your application will not be accepted until these situations are resolved. If a change to the registration information is needed at a later date, grantees may re-enter the system at https://grants.illinois.gov/registration/login.aspx .
No applications can be accepted for review until these steps are successfully accomplished. (Only after your entity has completed the above registration/pre-qualification steps will a state agency be allowed to accept your grant application for consideration, and that application must still be submitted by the respective due date.)
Grant Application, Program Plan (if applicable), Budget, and Programmatic Risk Assessment:
At the time of application, the grantee must submit the grant application, the program plan (if applicable), the budget, and the programmatic risk assessment. The application templates, the instructions, the due date, and the link to the programmatic risk assessment can be found on the DHS/DFCS Grant Information webpage under the CSFA number.
Fiscal and Administrative Risk Assessment:
Grantees must complete the Fiscal and Administrative Risk Assessment on the GATA/CSFA system- also known as the ICQ (short for Internal Controls Questionnaire). Be sure to click "submit" to submit your answers when complete. This is done only once per entity per fiscal year. While it does not have to be completed prior to submitting the application, this step must be done before an applicant or their application can be considered.
Indirect Cost Rate:
Most, but not all grants will include the potential for a grantee to claim an Indirect Cost Rate to help cover the administration costs related to the grant.
For new grantees or new grants, this must be done within 90 days of the grant's effective date.
For returning grantees, this must be done within six months following the end of the grantee's fiscal year.
All grantees must make a selection.
Grant Selection, NOSA and Grant Agreement:
If your application is selected for funding, the results of the ICQ and Programmatic Risk Assessment may render conditions that will be included in your Notice of State Award (NOSA). The NOSA will be generated from GATA's CSFA system, and may be delivered via email from that system. (if you change staff or email addresses, be sure to update your GATA registration with that information or the correct person or email will not receive the NOSA.) These conditions may need to be addressed prior to the award becoming a firm grant agreement or they may be addressed over the course of the timeframe of the award. The proposed IDHS grantee must sign-off and return the NOSA to IDHS. Once the grantee accepts the IDHS grant and returns it, IDHS will also accept and the contracting process begins.

An awardee is required to have a records retention policy. A records retention policy defines an organization’s procedures for the management, retention, transfer, and disposal of records. The policy must define the duration of time the awardee will preserve documents while maintaining them in accordance with applicable Federal and State retention requirements.
Financial records, supporting documents, statistical records, and all other awardee records pertinent to a State-issued award shall be maintained for at least five years after the end of the fiscal year to which they relate.

Program Accomplishments
The program has planned, coordinated, and hosted an annual IDHS Home Visiting Summit training event as well as the annual Statewide Child Abuse Prevention Conference for many years.
