Short Description
The Answer Inc. is an organization dedicated to improving and enriching the quality of life for families impacted by Autism /Developmental & Intellectual Disabilities by providing support, resources, education, recreation, advocacy, and workforce development. The goal is to de-stigmatize society’s opinion of people with Autism and Developmental Challenges and to empower and educate families living with Autism/Developmental & Intellectual Disabilities. They will achieve this goal through Parent & Community Trainings, Referral Services, Spectrum University (Tutoring for Young Adults), Music N Me (Exercise Nutrition & Dance Program), Bullying Intervention, 1st Responders Training, Horseback Riding Camp Scholarships, Therapeutic /Academic Scholarships and Social & Recreation Events.

Deliverables: 1. The grantee will provide a list of all participants in the program “Bullying is not The Answer”. a. Data Required: Provide the participant’s name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, and county they reside, and date of event attended. b. Provide the date of event, location, and topic of discussion. Due dates: Due dates: 1st Quarter Reports are due No Later Than (NLT) October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th. 2. The grantee will provide a list of meetings held for Father’s Support Group and Parent Support Group each quarter. a. Data Required: Provide total number of participants in each support group meeting. b. Provide date of meeting, location, and topic of discussion. Due dates: Due dates: 1st Quarter Reports are due No Later Than (NLT) October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th. 3. The grantee will provide a list of participants in the Academic Support Program. a. Data Required: Provide the participant’s name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, and county they reside. b. Provide date of academic support received, type of service received, location of service and name of person providing services and credentials. c. Provide individuals progress tracked (no recognition, emerging skill or skill mastered). Due dates: Due dates: 1st Quarter Reports are due No Later Than (NLT) October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th. 4. The grantee will provide a list of participants in the Exercise and Nutrition Program (Music ‘n’ Me Program). a. Data Required: Provide the participant’s name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, and county they reside. b. Provide date of program and topic of discussion. c. Due dates: Due dates: 1st Quarter Reports are due No Later Than (NLT) October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th. 5. The grantee will provide a list of participants in the Spectrum Socialite. a. Data Required: Provide the participant’s name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, and county they reside. b. Provide date of session and topic of discussion. Due dates: Due dates: 1st Quarter Reports are due No Later Than (NLT) October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th. 6. The grantee will provide a list of participants in the Spectrum Social. a. Data Required: Provide the participant’s name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, and county they reside. b. Provide date of outing and describe type of outing activity. Due dates: Due dates: 1st Quarter Reports are due No Later Than (NLT) October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th. 7. The grantee will provide a list of participants Training. a. Data Required: Provide the participant’s name, gender identity, last four of their Social Security Number (SSN#), Recipient Identification Number (RIN#), age, and county they reside. b. Provide date of training and type of training provided. Due dates: Due dates: 1st Quarter Reports are due No Later Than (NLT) October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th. 8. The grantee will provide a copy of the agency's survey and a summary of the responses from the Satisfaction Survey. Surveys should be distributed to the participant or the participant’s representative after event has been completed. a. Required data: Provide a blank copy of the survey for approval, the number of surveys distributed, and a summary of responses to each question listed on the survey. Due dates: 1st Quarter Reports are due NLT October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th.
