Short Description
The Illinois extensively drug-resistant organism (XDRO) registry is a dynamic infection surveillance and interfacility communication system that sends out real-time messages to healthcare facilities across the state. The XDRO registry is mandated by 77 Illinois Administrative Code Part 690, Subpart J. Please refer to the XDRO Registry site for further details: https://xdro.dph.illinois.gov/reporting-rule.html. The CDC has upheld Illinois’ registry as a national example to other states. IDPH seeks a time-limited direct grant agreement with an organization that will maintain the XDRO registry and website, collect XDRO data from healthcare facilities and labs, support user access, manage the registry’s automated alert system, and provide technical expertise to develop and modify the registry based on emerging needs. The grantee will also upgrade the registry platform to meet security standards and update it as needed. Infectious diseases disproportionately affect communities that are disadvantaged or at-risk (e.g., as a result of socioeconomic status, race/ethnicity, sexual orientation, gender, disability status, geographic location, etc.). The XDRO registry promotes health equity by serving as a broad patient-safety alert platform that informs Illinois’ healthcare facilities of their patient’s or resident’s XDRO status at the time of admission. Currently, it provides near real-time alerts to all of Illinois' acute care and critical access hospitals. Long-term care and other facility types must manually query the registry to obtain the same information. The grantee’s work will help to expand alert functionality to more healthcare facilities across the state, starting with nursing homes, and help these facilities share and act on interfacility communication to reduce XDRO transmission. The grantee will prioritize its engagement of facilities based on need, including facilities in areas with high XDRO rates and facilities serving disadvantaged communities. This work will help ensure that all at-risk populations are served by the registry. The grantee’s objectives will include the following starting from Month 1 of the grant: • Ensure XDRO registry operations. This includes supporting registry infrastructure, upgrading the underlying PHP: hypertext preprocessor script, collecting XDRO data from healthcare facilities and laboratories, and providing user support services (ensuring new users are enabled access to the registry, assisting users who are having difficulty accessing the registry or who need access to multiple facilities). • Manage the XDRO registry’s automated alert system. This includes establishing and validating data feeds from facilities and monitoring system operations. • Create and implement developments to the XDRO registry to address emerging public health needs, including automated alerts for skilled nursing facilities and for patients at high risk of having an XDRO. Strong applications will: (a) articulate how proposed activities align with national and state priorities (b) show how the applicant, as the registry’s technical expert, will be compliant with the state mandate (see above) and meet all required elements (c) demonstrate understanding of, and collaboration with, key stakeholders (d) articulate how the proposed activities will promote health equity (e) include a clear evaluation plan, and (f) address the other requirements noted in the Application Review Criteria section below. The Illinois Department of Public Health places health equity as a top priority. Health equity is the “basic principle of public health that all people have a right to health”. Health equity exists when all people can achieve comprehensive health and wellness despite their social position or any other social factors/determinants of health. Most health disparities affect groups marginalized because of socioeconomic status, race/ethnicity, sexual orientation, gender, disability status, geographic location, or some combination.

Objective
The purpose of this grant will be to maintain an extensively drug resistant organism (XDRO) registry, which acts as a dynamic infection surveillance and interfacility communication system that sends out real-time messages to healthcare facilities across the state. The grantee will collect XDRO data from healthcare facilities and labs, maintain the registry, validate data feeds, educate and assist facilities on reporting and using the XDRO data to improve interfacility communication and infection prevention efforts at their facilities. The grantee will also independently prioritize facilities for education and technical assistance, based on gaps they identify from reviewing data

Strong applicants will have pre-existing relationships with healthcare facilities across the state and the capability to independently onboard and/or provide education and technical assistance to facilities based on data collected by the platform.

Criteria Selecting Proposals
Need (5 points): Does the applicant provide data, facts, and/or evidence that demonstrate that the proposal supports the grant program purpose? Capacity (20 points): (a) (10 points) Does the applicant demonstrate its ability to execute the grant project according to project requirements? (b) (10 points) Does grantee have experience in the last five years developing or implementing activities related to XDRO surveillance and data collection at a regional or statewide level? Quality (10 points): Does the applicant demonstrate that the project, in total, is well articulated and in alignment with the project requirements? Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b. Appeals must be received within 14 calendar days after the date that the grant award notice was published. Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request. The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required.

See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.

Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
