For the Scope of Work, under Project Goals, each applicant must list at least one or more ordinary and contingent expense. If the Budget includes cash or in-kind amounts, the source of those funds must be identified. Grantees must also submit or upload other required documents and attachments, including but not limited to, organization W-9 form and Sub-Contractor/Sub-Grantee Authorization form(s). Grantees shall receive a one-year executed grant agreement for the grant program applied for in the EGrAMS applications.

The Legislative Initiative 2023 Alton Memorial Hospital program requires quarterly Financial Reports.
