Objective
The grantee, Suburban Primary Health Care Council, will receive the appropriation from the General Revenue Fund for Access to Care operational expenses to enable approximately 758 low-income, uninsured, and underinsured individuals who are not eligible for Medicare or Medicaid to join the Access to Care program, at the 2022 per person cost of $659.00.

Preapplication Coordination
All pre-award requirements must be met.
Application Procedures
The application will be reviewed by the Department grant monitor. Upon final application approval, a grant agreement will be sent to the applicant for partial execution. When returned to the Department, the agreement will be processed and fully executed. A final copy will be forwarded to the applicant.
Criteria Selecting Proposals
Adequate work plan and budget to meet the overall goals of the program. Non-competitive application will be sent for revision if criteria have not been met.
Award Procedures
Grant may be renewed annually depending upon allocations by the Illinois legislature. Grantee must submit a work plan with detailed objectives and timelines and annual budget. This grant is non-competitive. Application review and approval will be completed by the IDPH grant monitor based on prior grant guidelines, timely submission of reports and fiscal monitoring.

Deadline date is the same date as the end date on NCFI at 5:00 p.m.
Range of Approval or Disapproval Time
10 working days after application closes.
Appeals
The appeals process is only available for competitive grants on the evaluation process.
Renewals
The grant may be renewed if continuing funds are allocated by the Illinois legislature.
Formula Matching Requirements
This grant does not have any formula or matching requirements.
Uses and Restrictions
All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for recouping of those funds used for the prohibited purpose. Expenditure reports must be submitted quarterly.

Recipients are to follow audit requirements per JCAR Title 44 Illinois Administrative Code 7000.90.
