Short Description
The HIV Self Testing Grant shall increase the accessibility of HIV testing for populations most affected by HIV through the implementation or expansion of HIV Self-Testing (HIVST) programs in Illinois while building the capacity of community-based organizations (CBO). The outcomes of this demonstration project will be used to assess the efficacy of such a program by reviewing the successes and challenges of providing self-test kit options, to include linkages to care or additional prevention services. At the conclusion of the project, grantees will be expected to participate in a presentation of outcomes, successes, and challenges.

Objective

At the conclusion of the project, grantees will be expected to participate in a presentation of outcomes, successes, and challenges.

1) Grantee will implement a pilot or expand an existing HIVST program. 2) Grantee will develop or maintain and expand an agency protocol for the HIVST program, to include training of identified staff on how to respond to clients utilizing a status neutral approach for referring clients to medical care or prevention services such as PrEP or nPEP within the first 30 days of the performance period. 3) Grantee will develop or maintain a resource for status neutral referrals within the first 30 days of the performance period. 4) Grantee will develop a survey tool for clients regarding the HIVST and follow up services within the first 30 days of the performance period to include questions such as but not limited to the following: a. Why did you choose to request an HIV self-test? b. How satisfied were you with the request process? c. How satisfied were you with the time it took to receive the test kit? d. How satisfied were you with the guidance received from the agency providing the test kit? e. Did you choose to report your results to the agency? i. If no, why not? ii. If yes, did you receive information for follow up or other resources? f. Do you plan to access any of the services suggested by the agency? 5) Grantee will develop and provide a presentation on the pilot program’s lessons learned, challenges, successes, and outcomes. Presentation will be scheduled by IDPH no later than August 31, 2023. 6) Grantee will develop marketing materials to support HIVST, submitting them to IDPH for approval prior to use. Grantee will identify the most appropriate model of delivery of HIVST kits for their clients, providing both an in-person and mail option.

Applicant organizations may be local health departments, community health centers/federally qualified health centers (FQHC) or not-for-profit community-based organizations, including volunteer or religious organizations.

Application Procedures
Application must be submitted through EGrAMS (idphgrants.com). All sections of the application must be completed including the budget within EGrAMS. The application must be validated, certified, and submitted through EGrAMS.
Criteria Selecting Proposals
An objective team review and evaluation process will be conducted to determine grant award recipients. Applications with the highest consolidated panel scores will be selected for funding. Scoring is based on the following categories: a) Organizational capacity and experience (7 points) b) Project Description (22 points) c) Project Management and Staffing (6 points) d) Work Plan (20 points) e) Health Equity (35 points) f) Project Budget Narrative (10 points)
Award Procedures
Awarded applicants will be notified via email from EGrAMS. A Notice of State Award (NOSA) shall be issued to the finalists who have successfully completed all grant award requirements and have been selected to receive grant funding. The NOSA will specify the funding terms and specific conditions resulting from applicable pre-award risk assessments. The Illinois Department of Public Health (IDPH) is exempt from utilizing the standard NOSA issued on the GATA Grantee Portal. Successful applicants will receive an email notification from EGrAMS and must review the funding terms and specific conditions in the grant agreement and accept utilizing an electronically signature. Both the electronic signature in EGrAMS and a physical signature on the grant agreement must be completed by an authorized representative of the grantee organization and submitted to IDPH. A Notice of Denial shall be sent to the applicants not receiving awards via EGrAMS.

Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the GATA section of the IDPH website (www.dph.illinois.gov/GATA). o Appeals must be received within 14 calendar days after the date that the grant award notice was published. o Appeals must include the following information: ? The name and address of the appealing party ? Identification of the grant ? A statement of reasons for the appeal ? If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. o The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. o The appealing party must supply any additional information requested by the agency within the time period set in the request. o The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required. Documentation of the appeal determination shall be sent to the appealing party and must include the following: o Standard description of the appeal review process and criteria o Review of the appeal o Appeal determination o Rationale for the determination In addition to providing the written determination, the grant-making office may do the following: o Document improvements to the evaluation process given the findings and re-review all submitted applications. o Document improvements to the evaluation process given the findings and implement improvements into the following year's grant evaluation process. o Provide written notice to the appealing party as to how the identified actions will be remedied. Appeals resolutions may be deferred pending a judicial or administrative determination when actions concerning the appeal have commenced in a court of administrative body.

Uses and Restrictions
he grant funds may not be used for institutional, organizational, or community-based overhead costs not associated with the HIV Self-Testing Program, or levies.
Reports
• Grantee shall submit quarterly progress reports addressing the following: • Number of at home test kits procured for distribution. • Number of at home test kits provided to clients. • Number of risk materials provided to clients. • Number of clients who report no previous HIV testing (prior to HIVST). • Number of clients who report previous HIV testing (prior to HIVST). • Number of clients who contacted the grantee to report their HIVST results. • Number of clients contacted by the grantee to obtain HIVST results. • Number of clients referred to care after reporting a reactive HIVST. • Number of clients referred to PrEP after reporting a non-reactive HIVST. • Number of clients linked to care. • Number of clients linked to PrEP. • Grantee shall punctually submit an interim progress report in the format provided by the HIV/AIDS Section on April 15, 2023. Grantee shall punctually submit a final, closeout report in the format provided by the HIV/AIDS Section on the last day of the month following the end of the grant period.
