Short Description
The purpose of this funding opportunity is to educate the State of Illinois’ Regional Health Care Coalitions, Frontline Hospitals, designated Special Pathogens Assessment Hospitals, and Special Pathogens Treatment Centers (ETC) to identify, diagnose, and treat a patient with suspected Ebola or any other High Consequence Infectious Disease that may impact the healthcare system. All frontline healthcare facilities must be able to identify, diagnose, and treat a patient with suspected Ebola until the person(s) under investigation can be transferred to a facility that can provide definitive care. This funding aims to ensure the health care system in Illinois is well prepared in the event of Ebola or other High Consequence Infectious Disease outbreaks.

Objective
Scope of Work and Activities: 1. The grantee shall ensure the acute care hospital designated as a Special Pathogens Assessment Hospital satisfies all requirements outlined in the Health Care Facility Special Pathogen Preparedness Checklist and CDC’s Interim Guidance for Preparing Ebola Assessment Hospitals. The grantee will submit proof of capabilities by submission of a checklist in the format specified by IDPH. 2. The grantee shall submit a High Consequence Infectious Disease (HCID) facility plan to the Department that outlines the ability to quickly identify, isolate, test, and treat a patient with a special pathogen such as Ebola disease for up to 96 hours and arrange transport to a special pathogen treatment center. This plan will also address healthcare worker team membership, draft staffing schedules, and communication plans. 3. The grantee shall maintain healthcare worker readiness by providing at least 1 (one) training per quarter and maintaining training rosters, which shall be submitted to the Department upon request. At least two (2) trainings per year must be in person for assigned healthcare staff. Training objectives should, at a minimum, cover identification, isolation, treatment, laboratory testing processes, infection prevention, Personal Protective Equipment (PPE) donning/doffing, and waste management. 4. The grantee shall use at least 40% of the grant budget to maintain appropriate levels of PPE for the facility to provide care for up to 96 hours from receipt of a patient under investigation for a special pathogen. 5. The grantee will establish a transport agreement with an Emergency Medical Services Advanced Life Support (ALS) Provider for the transport of an individual under investigation for a special pathogen, such as Ebola. 6. The grantee will allocate at least 10% of the grant budget to the purchase of materials or equipment, such as isopods or drapes, to the EMS provider to prepare the assigned transport vehicle for these patients and train their staff how to prepare and transport safely. 7. The grantee shall obtain at least four (4) jump bags with specified materials provided by the Department, such as PPE for regional hospitals to utilize in the case of the receipt of a patient under investigation. 8. The grantee shall participate in site visits or trainings as requested by the Department. 9. The grantee shall adjust or augment its electronic health records (EHR) system to align with CDC recommendations to quickly identify and isolate patients under investigation for a special pathogen. The grantee shall also encourage regional hospitals to incorporate travel screenings or symptom identification into their EHR. 10. The grantee will provide quarterly progress reports in a format specified by the Department that includes, at a minimum, trainings provided, participation rosters, and currently designated patient care team membership.

Only Illinois Licensed hospitals in Emergency Medical Services Regions 1 thru 10 for the designation of Special Pathogens (Ebola Assessment Hospitals) and designated Special Pathogens (Ebola Treatment Centers) located in EMS Region 11 may apply for this grant. An entity may not apply for a grant until the entity has registered and pre-qualified through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, Grantee Links tab. Registration and pre-qualification are required annually. During pre-qualification, verifications are performed including a check of Federal Debarred and Suspended and status on the Illinois Stop Payment List. An automated email notification to the entity alerts them of “qualified” status or informs how to remediate a negative verification (e.g. inactive Dun and Bradstreet, not in good standing with the Secretary of State). A federal Debarred and Suspended status cannot be remediated.
