Short Description
The Illinois Department of Public Health (IDPH) was awarded CDC funding though the Cooperative Agreement for Emergency Response: Public Health Crisis Response. CDC expects that at least 25% of the jurisdictional award will support school-based health programs focusing on increasing the physical, emotional, and mental wellbeing of youth across Illinois. The Office of Women’s Health and Family Services though the School Health Program is providing funding for Illinois school districts to purchase telehealth supplies, equipment, and if needed hire a mental health care professional.

Objective
The Enhancing Telehealth Post-Covid for Adolescent Health Needs seeks to provide one time funding to Illinois K-12 school districts, local health departments (LHD), federally qualified health centers (FQHC), rural health centers (RHC), and other educational institutes for telehealth related resources that will provide increased workforce availability for mental health services to school aged youth (medical services may also be provided). These funds are to support school-based healthcare for youth and aid the community in accessible care to mental health providers for youth. Any entity applying that is not a school district or private school serving youth must partner with an Illinois school district to be eligible (a letter of partnership must be included in the application). The range for each grant award is up to $100,000.00 per grantee. If the grantee is serving more than one school district, then they may only partner with a maximum of two school districts (Must submit 2 separate grant applications). Program Objectives: • Enhance workforce ability to provide mental health support programs within Illinois K-12 schools through increased outreach and resources for students. • Reduce health disparities in mental health due to lack of access to providers, transportation barriers, and other social determinants of health. • Acquire equipment and supplies needed for telehealth services.

Application Procedures
The application format is provided via EGrAMS as mandated for all Illinois Department of Public Health Grants. The application MUST be completed in its entirety by the deadline listed. This includes submission of all mandatory forms required for all agencies to include letters of partnership. Applicants MUST submit a completed application including all required documents via EGrAMS by the deadline. There is a two-step submission process in EGrAMS. Criteria Selecting Proposals
An objective and merit-based grant review and evaluation process will be conducted in EGrAMS to make the final program funding determinations and grant awards This is a competitive grant. Agencies identified in Section C of this Notice of Funding Opportunity are eligible to apply for funding but not all applicants will receive an award. Applications scoring the highest will receive funding at the Department’s discretion.
Award Procedures
Upon completion of the submission and review process, each successful grantee will receive a grant agreement to be signed by the entity’s authorized official. The grant agreement is not binding on the parties until it has been fully executed by the Illinois Department of Public Health.

Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the GATA section of the IDPH website (www.dph.illinois.gov/GATA). o Appeals must be received within 14 calendar days after the date that the grant award notice was published. o Appeals must include the following information: ? Formula Matching Requirements
Matching funds (cash/in-kind contributions) are not required by the applicant. If applicant receives cash/in-kind contributions, these need to be included in the total operating budget reported in the online grant management system and specified in the grant budget summary.
Uses and Restrictions
The Grantee will expend funds awarded under this agreement in accordance with the budget approved and on file with the Department. Departmental approval of a budget, including subcontractors or subgrantees, does not constitute written consent for the use of such services. The Grantee will not commingle funds between separate grants or subgrants, even if the grants or subgrants are related or the same population is being served. The Grantee and any approved subgrantees or subcontractors shall not expend any funds for promotional items unless the Department’s grant manager has given written permission to do so. Promotional items include, but are not limited to: calendars, pens, buttons, pins, magnets, gift cards, posters, and stationery. State of Illinois Grant Awards will not allow reimbursement of pre-award costs.
Reports
Monthly Reimbursement Forms will be due on the 15th of the following month; Quarterly Progress Reports will be due on the 30th of the month following the closure of the quarter; and End of Grant Report will be due forty-five days from the last date of the grant period. All reports and monthly reimbursement forms shall be submitted through the online grants management system.

Records
Grantees are required to maintain grant accounting records 3 years after the date they submit the Federal Financial Report (FFR). If any litigation, claim, negotiation, audit or other action involving the award has been started before the expiration of the 3-year period, the records shall be retained until completion of the action and resolution of all issues which arise from it, or until the end of the regular 3-year period, whichever is later.
