Short Description
Poor nutrition and low levels of physical activity can impact overall health and are considered significant risk factors for obesity and other chronic diseases. Prior to the COVID-19 pandemic, obesity impacted 19.3% of children ages 2 to 19 years, which is roughly 14.4 million children and adolescents (CDC, 2022). During the course of the pandemic, changes in dietary behaviors, increased unhealthy food choices (e.g., potatoes, sugary drinks, highly processed foods), food insecurity linked to financial reasons, and the restrictions/limitations imposed on out of the home physical activities all represented risk factors involved in weight gain, lack of physical activity, and health concerns in school aged children.

Objective
Grantees will utilize evidence-based strategies along with developed partnerships/stakeholders (e.g., local schools, communities, health departments, and community-based organizations) to change school and community environments by facilitating change (e.g., workforce enhancement/educational trainings to student body as well as school workforce), and creating an atmosphere that promotes positive, sustainable lifestyle changes for the target population. The trainings offered will help to support overall mental and emotional wellbeing of children/adolescents and reduce the need for preventable disease resources. Ultimately, not only will this have a positive impact on the healthcare workforce, but the future of the children/adolescents impacted by this program

Eligible applicants are limited to those agencies that are Illinois based, local health departments (LHD), federally qualified health centers (FQHC), and rural health centers (RHC) Community Based Organizations, Community groups/centers with 501-C3 status, School/ School Districts and Colleges or Universities. This grant program is competitive, and funding is limited. Awards will be based on scores and available funding. It is important to note that all applicants may not receive an award

Applications will be reviewed and scored by IDPH OWHFS staff for completeness and accuracy as well as the criteria identified below: 1. Criteria The following components must be submitted by all applicants to be considered for funding: • Applicant Organization Information • Applicant Grant History • Grant Project Proposal (Scope of Work) • Staff and Organizational Experience (Scope of Work) • Program Work Plan* • Budget Justification • Applicant Certification • W-9 Form *Work Plan must provide objectives/goals with measurements and expected outcomes. Strategies must be Specific, Measurable, Achievable, Realistic, Timely, Inclusive and Equitable (SMARTIE). • Specific – Make your goals specific and narrow for more effective planning. • Measurable – Define what evidence will prove you’re making progress and reevaluate when necessary • Achievable – Make sure you can reasonably accomplish your goal within a certain timeframe. • Realistic – Your goals should align with your values and long-term objectives. • Timely – Set a realistic end date for task prioritization and motivation. • Inclusive – It includes those most impacted—into processes, activities, and decision/policymaking in a way that shares power. • Equitable – It includes an element of fairness or justice that seeks to address systemic injustice, inequity, or oppression. (Outcomes and measurements MUST align with the stated activities) Grants will be reviewed by program staff during a competitive review process, based on a 100-point scoring rubric. The categories are as follows: Grant History: 10 points Scope of Work: 25 points Work Plan: 25 points Budget: 5 points Health Equity checklist: 35 points.
