The Child/Adolescent Preventing ACEs After COVID Program seeks to identify mental health issues and needs within the target population and provide training and education, resources, care coordination, etc. to improve the mental health outcomes and build resiliency for children and youth in a post pandemic world.

Objective
• Utilizing evidence-based research to develop trainings and tools for health professionals/providers, schools/school districts, and communities. • Support evidence-informed guidance, to address behavioral, social, and environmental determinants of health. • Assisting schools and communities in raising awareness on ACEs education/training, trauma, and resiliency. • Assisting schools/school districts in the screening, identification and proper referral for ACEs/mental health issues experienced by the student body. • Assisting providers in adopting a more patient-centered care approach that incorporates screening/referral for ACEs/mental health issues and provides health care in a clinical setting that meets the needs of children during their yearly well care visits. • Leveraging existing programs and funding targeting ACEs (physical and mental health). • Assist schools/communities with assessing the post COVID barriers children/adolescents face with ACEs/mental health issues and access to care/help. The first six (6) months of this grant will be designated for a: (1) Planning Phase – Grantees must develop a plan to partner in their community to achieve grant program goals. The last twelve months (12) of the grant will focus on: (2) Implementation Phase – Grantees must implement the plan they developed to assist identified program partners with awareness and training, and children/adolescents with identified ACEs mental health issues, access to care, and appropriate level of care referrals.

Applicant Eligibility
Eligible applicants are limited to those agencies that are Illinois based, local health departments (LHD), federally qualified health centers (FQHC), and rural health centers (RHC) Community Based Organizations, Community groups/centers with 501-C3 status, School/ School Districts and Colleges or Universities. This grant program is competitive, and funding is limited. Awards will be based on scores and available funding. It is important to note that all applicants may not receive an award.

Preapplication Coordination
All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active DUNS number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State. Criteria Selecting Proposals
Applications will be reviewed and scored by IDPH OWHFS staff for completeness and accuracy as well as the criteria identified below. 1. Criteria The following components must be submitted by all applicants to be considered for funding: • Applicant Organization Information • Applicant Grant History • Grant Project Proposal (Scope of Work) • Staff and Organizational Experience (Scope of Work) • Program Work Plan* • Budget Justification • Applicant Certification • W-9 Form *Work Plan must provide objectives/goals with measurements and expected outcomes. Strategies must be Specific, Measurable, Achievable, Realistic, Timely, Inclusive and Equitable (SMARTIE). • Specific – Make your goals specific and narrow for more effective planning. • Measurable – Define what evidence will prove you’re making progress and reevaluate when necessary • Achievable – Make sure you can reasonably accomplish your goal within a certain timeframe. • Realistic – Your goals should align with your values and long-term objectives. • Timely – Set a realistic end date for task prioritization and motivation. • Inclusive – It includes those most impacted—into processes, activities, and decision/policymaking in a way that shares power. • Equitable – It includes an element of fairness or justice that seeks to address systemic injustice, inequity, or oppression. (Outcomes and measurements MUST align with the stated activities) Grants will be reviewed by program staff during a competitive review process, based on a 100-point scoring rubric. The categories are as follows: Grant History: 10 points Scope of Work: 25 points Work Plan: 25 points Budget: 5 points Health Equity checklist: 35 points

Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
