Short Description
The program will provide logistical support for moving to new homes, support for well-being during resettlement transitions, community care & shelter support, and donation receiving and distribution for southwest border arrivals as families are in intermediate housing, tirage shelter, police stations, or other temporary shelter on the journey to the housing of their choice and in the communities of their choice. 1. Logistical support with moving to new homes. This will include transportation to visit units and/or furniture appointments. On-site assistance on moving day and purchasing and delivering furniture and home goods for the families. 2. Shelter operations and community care support to help families transition into stable living situations. Providing safe spaces for residents in shelter and opportunities to develop new interests, build friendships and community, find their place in a group, and gain developmentally relevant experiences. 3. Soliciting Care Items and Donations – coordinating across the City, State, and private partners (including other NGOs, corporations) to source donations, with a particular focus on care items. -Receive and sort donations. -Distribute care products and donations to new arrivals -Documenting/maintaining records of purchases/donations, available items, and distributed items.

Reports
Reporting Requirements: A. Time Period for Required Periodic Financial Reports. Unless a different reporting requirement is specified in Exhibit E, Grantee shall submit financial reports to Grantor pursuant to Paragraph 10.1 and reports must be submitted no later than 30 days after the quarter ends. B. Time Period for Close-out Reports. Grantee shall submit a Close-out Report pursuant to Paragraph 10.2 and no later than 30 days after this Agreement's end of the period of performance or termination. C. Time Period for Required Periodic Performance Reports. Unless a different reporting requirement is specified in Exhibit E, Grantee shall submit Performance Reports to Grantor pursuant to Paragraph 11.1 and such reports must be submitted no later than 30 days after the quarter ends.
