Short Description
Public Act 103-0006, Article 56, Section 675. The sum of $14,810.60 or so much there of as may be necessary is appropriated from the General Revenue Fund to the Department of Human Services for grants and administrative services associated with violence prevention programs, youth employment programs, and operational expenses to the following named entities, in the approximate amounts below:

Objective
GAP Community Center is a safe haven for numerous immigrants that migrate to the US from Central America as well as the Caribbeans. Our goal is to allow adults and children to excel in our community by providing them with their essential needs. In 2022, when our government opened the borders, GAP Community Center partnered with the City of Chicago to provide emergency services for these families and individuals. Although the City of Chicago was unable to provide shelter for the immigrants, we were fortunate enough to render them with access to our facility at the Revive Center which provided not only a temporary place of security, but also warm showers, clothing, and hot meals. Upon arrival, we studied the needs of each individual and concluded that their needs were much greater than our initial assessment. Within the first 24 hours of our interactions with each person, we were able to offer assistance with multiple services that would be of great benefit to each person individually. Because of the hardships they had to endure while traveling, we observed that there was a great need for medical assistance, housing, and education for both adults and children. Between the months of August and October, Gap Community Center was able to provide over 2,000 immigrants with their essential needs such as rental assistance, public aid, medical care and education.
Currently, we are still able to assist multiple immigrants with these services that come to our facility on a weekly basis and we hope that you will join us in saving more lives in the many
years to come. Services we assist: Food Hygiene products Clothes: T-shirts, Pants, Shoes undergarments etc. School for children, needs assessment, Public aid application, medical appointments, immigrant appointments, assist with locating legal aid.

Renewals
This Agreement may be renewed for additional periods by mutual consent of the
Parties, expressed in writing and signed by the Parties. Grantee acknowledges that this Agreement does not create any expectation of renewal.
Uses and Restrictions

Reports
A. Time Period for Required Periodic Financial Reports. Unless a different reporting requirement is specified in Exhibit G, Grantee shall submit financial reports to Grantor pursuant to Paragraph 13.1 and reports must be submitted no later than 15 days after the quarter ends.
B. Time Period for Close-out Reports. Grantee shall submit a Close-out Report pursuant to Paragraph 13.2 and no later than 45 days after this Agreement's end of the period of performance or termination.
C. Time Period for Required Periodic Performance Reports. Unless a different reporting requirement is specified in Exhibit G, Grantee shall submit Performance Reports to Grantor pursuant to Paragraph 14.1 and such reports must be submitted no later than 15 days after the quarter ends.
D. Time Period for Close-out Performance Reports. Grantee agrees to submit a Close-out Performance Report, pursuant to Paragraph 14.2 and no later than 45 days after this Agreement's end of the period of performance or termination.
Grantee shall submit quarterly Periodic Performance Report (GOMBGATU-4001 (N-08-17)).
Grantee shall submit a quarterly Periodic Financial Report (GOMBGATU-4002 (N-08-17)). Reported expenses should be consistent with the approved annual grant budget. Any expenditure variances require prior Grantor approval in accordance with Article VI of the Uniform Grant Agreement to be reimbursable.
Audits
Grantee shall be subject to the audit requirements contained in the Single Audit Act
Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor's Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.334, unless a different retention period is specified in 2 CFR 200.334 or 44 Ill. Admin. Code 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the records must be retained until all litigation, claims or audit exceptions involving the records have been resolved and final action taken.
