Short Description
The Illinois Department of Public Health (IDPH) Dementia Program will provide general revenue funds to develop and deliver education and support for caregivers of people with Alzheimer’s Disease and Related Disorders (ADRD) from four diverse groups: Component 1-Black Americans, Component 2-Hispanic/Latinx Americans, Component 3-Asian Americans, and Component 4-Rural Caregivers. The purpose of the Dementia Caregiver Program for Underserved Populations grant is to increase awareness, education, support, and community connection for caregivers of people living with ADRD in order to reduce stress, empower, and support caregivers from diverse populations, as well as reduce barriers and stigma that prevent access to care. This new grant program aims to support strategies from the 2023-2026 Illinois Alzheimer’s Disease State Plan, including to identify and partner with community-based organizations to assist in the dissemination of culturally responsive caregiving resources and support (Public Education and Awareness 2.E.6) and to promote awareness and use of evidence-based caregiver education (Service Delivery 3.D.3.a). Many of the activities in this program will also align with the Centers for Disease Control and Prevention’s Healthy Brain Initiative, including to Ensure caregivers have information, tools and resources about their vital role and ways to maintain their own health and well-being, Action E-7 from the Healthy Brain Initiative: State and Local Road Map for Public Health, 2023-2027 (cdc.gov).

Objective
The granted organization will educate caregivers about the diagnosis of Alzheimer’s Disease and Related Disorders (ADRD), best care practices, culturally appropriate community resources, and increase the caregiver’s level of confidence when advocating and discussing ADRD with healthcare professionals on behalf of care recipients. Grantees will recruit caregivers from the targeted communities listed to participate in the program. Grantees will provide all necessary materials to educate caregivers including but not limited to Wi-Fi and technical support, electronic device access (i.e., borrowing system), multilingual ADRD caregiver handbooks, and any other materials deemed necessary to educate caregivers. The granted organization will be responsible for promoting the trainings to the targeted audience of caregivers identified, registering and on-boarding the attendees for all trainings, creating and providing the educational content, facilitating the training events, administering pre and post-tests, arranging online support groups, and setting up in-person memory cafes to be offered to caregivers and care recipients with ADRD.

Applicant Eligibility
Eligible applicants include a public or private not-for-profit entity, or profit entity capable of developing a dementia curriculum that reflects high quality, best practiceinformed content from both the dementia and EMS fields. The Department encourages diverse applicants and organizations to apply. The grantee may apply for this grant but will not be eligible for a grant award until they are pre-qualified through the Grant Accountability and Transparency Act (GATA) Grantee portal, www.grants.illinois.gov. During pre-qualification, Dun and Bradstreet verifications are performed including a check of Debarred and Suspended status and good standing with the Secretary of State. The pre-qualification process also includes a financial and administrative risk assessment utilizing an Internal Controls Questionnaire. If applicable, the entity will be notified that it is ineligible for award as a result of the Dun and Bradstreet verification. The entity will be informed of corrective action needed to become eligible for a grant award.

Preapplication Coordination
All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active DUNS number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State. Application Procedures
Applications must be submitted via the Illinois Department of Public Health's Electronic Grants Administration and Management System (EGrAMS), accessible at www.idphgrants.com This work will occur between October 1, 2023 and June 30, 2024. The applicant must submit a project plan that describes how the award will be executed. The project plan should include necessary detail to enable the agency to manage the grant agreement activity against planned project performance. The grant application shall include information about the following: • Description of program capability by explaining the applicant’s expertise in developing ADRD training curricula for physicians and other healthcare professionals, establishing or improving medical clinic procedures and outcomes. • Detailed description/information about the proposed project. • Describe how this program will be implemented including expected deliverables and project requirements. • List of goals to be accomplished during the grant period. • Outline objectives by month with a list of tasks that will be implemented to accomplish the objectives (Work Plan.) The organization shall specify how the objectives will be measured to determine successful completion. • Outline the personnel supported by the grant. Include name and title of all staff participating on the project, percent of time projected that will be reimbursed through the grant funding. Include resume of project director as attachment. • Detailed budget by line item and justification. Risk Assessment Questionnaire Information (completed within EGrAMS) - In response to the requirements of 2 CFR 200.205, the awarding agency is required to review the programmatic risk posed by applicants. Four risk categories are assessed through this questionnaire: 1. Quality of management systems and ability to meet the management standards; 2. History of performance; 3. Reports and findings from audits performed under Subpart F—Audit Requirements of this part or the reports and findings of any other available audit; and 4. The applicant's ability to effectively implement statutory, regulatory, or other requirements imposed on awardees. Please note that a Health Equity Checklist Questionnaire is also required in EGrAMS, and will make up 35% of the total scoring for the grant application. Progress report requirements – grantees are required to submit quarterly progress reports on their work plan objectives. Criteria Selecting Proposals
This is an allocation grant that will provide awards from the Illinois General Revenue Fund to the grantees previously awarded in FY24. This grant is non- competitive. An individual review of the application will be conducted.

Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b. Appeals must be received within 14 calendar days after the date that the grant award notice was published. Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request. The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required..
Renewals

Uses and Restrictions
All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for recouping of those funds used for the prohibited purpose. Expenditure reports must be submitted quarterly. To be reimbursable under the Department/Office of Health Promotion Grant Agreement, expenditures must meet the following general criteria: •Be necessary and reasonable for proper and efficient administration of the program and not be a general expense required to carry out the overall responsibilities of the applicant. • Be authorized or not prohibited under federal, state or local laws, or regulations. •Conform to any limitations or exclusions set forth in the applicable rules, program description, or grant agreement. •Be accorded consistent treatment through application of generally accepted accounting principles, appropriate to the circumstances. •Not be allocable to or included as a cost of any state or federally-financed program in either the current or a prior period. •Be net of all applicable credits. •Be specifically identified with the provision of a direct service or program activity. •Be an actual expenditure of funds in support of program activities, documented by check number, and/or internal ledger transfer of funds. •Not be used for research or clinical care.
Reports
The grantee will be required to submit quarterly progress reports on their work plan objectives and quarterly reimbursement certifications within 30 days after the reporting period by e-mail (with scanned signature) or through the EGrAMS system: Failure to submit required reports in a timely manner will result in delays with approval of reimbursements. The final report and reimbursement certification are required to be submitted by July 31, 2024.
Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
