Short Description
Provides free healthcare services such as care coordination, case management, wellness education, primary care, specialty care, therapy (group/individual), dental care, free medication, and emergency shelter to Will and Grundy County residents in Illinois who are uninsured, underinsured, housing insecure or homeless, and are at or below the federal poverty guidelines, lack medical access, and lack a medical home.

Objective
Empowers individuals, families, and communities to improve their health in interconnected areas (biological, behavioral, social, and environmental) by using an innovative model of care to better address complex health needs present in underserved patient base which includes screening tools and health risk assessments that incorporate medical, behavioral, and social factors, care coordination, timely interventions and streamlined care transitions.

Direct Services - Educate all patients about potential benefits and health insurance - Assist all participants to begin applications into eligible benefits and health insurance (including Supplemental - Security Income/Social Security Disability Insurance, Temporary Assistance for Needy - Families, Supplemental Nutrition Assistance Program, etc.) - Utilize social determinants of health screening tools - Connect clients with high risk screening scores to mental health services, substance use treatment and/or case management - Link participants in need of behavioral health or substance use services to such services - Provide on-site or telehealth therapy services to interested participants - Provide linkage services (through committed partner agencies or the Iris referral platform) for recovery support services, childcare, transportation, rental assistance, life skills, vocational training, and/or any other services not offered from WGMC) - Provide medication teaching to patients taking prescribed medication Quality Assurance: - Annual training in data quality, record-keeping, evidence-based intake practices (including motivational interviewing), cross-cultural communication, and conflict resolution

Preapplication Coordination
Applicants must submit a completed Universal Grant Application as well as complete the pre-qualification processes. Each applicant (unless the applicant is an individual or Federal or State awarding agency that is exempt from those requirements under 2 CFR § 25.110(b) or (c), or has an exception approved by the Federal or State awarding agency under 2 CFR § 25.110(d)) is required to: be registered in SAM before submitting the application; provide a valid EUI number in its application; and continue to maintain an active SAM registration with current information at all times during which the applicant has an active Federal, Federal pass-through or State award or an application or plan under consideration by a Federal or State awarding agency.
