Short Description
The purpose of the five-year project is to improve transition outcomes, competitive integrated employment (CIE), and independent living outcomes for students with disabilities ages 10- 24 across the State. To that end, this project will develop a replicable and scalable innovative service delivery model that builds a seamless, cohesive, and effective required partnership between the Division of Rehabilitation Services, the Illinois State Board of Education, Centers for Independent Living (Access Living, RAMP, LIFE, Lake County, DRC, Options, IMPACT, and Southern Illinois), Local Educational Agencies (Minooka CHSD 111, Collinsville CUSD 10, Murphysboro CUSD 186, Chicago Public Schools District 205, Iroquois County CUSD 9, Waukegan Public School SD 60, and Prairie Central CUSD 8), Illinois State University, University of Illinois, and the Illinois Institute of Technology. The proposed innovative pre-ETS model program will employ a comprehensive system of integrated resources, trainings, technical assistance, and supports utilizing coordinated expertise from all required partners that will focus on the following three major domain areas: 1) Pre-ETS to CIE, (2) independent living, and (3) professional development (Walters & Plottner, 2023). The proposed project will also work with high demand employers and other community and state agencies to maximize the implementation of the statewide model and build statewide capacity and partnerships to support CIE and independent living for Illinois students with disabilities.

Reports
(1) FINAL REPORTS - ALL RECIPIENTS are required to submit a final performance report within 120 days after the expiration or termination of grant support in accordance with submission instructions provided in box 10 of the Grant Award Notification (GAN), or through another notification provided by the Department of Education (Department) (2 CFR § 200.329(c)). (2) ANNUAL, QUARTERLY, or SEMIANNUAL REPORTS - ALL RECIPIENTS of a multi-year discretionary award must submit an annual Grant Performance Report (34 CFR § 75.118). The annual performance report shall provide the most current performance and financial expenditure information that is sufficient to meet the reporting requirements of 2 CFR §§ 200.328, 200.329, and 34 CFR § 75.720.
