Short Description
The Illinois Department of Public Health’s Office of Women’s Health and Family Services (OWHFS) strives each year to improve maternal, child, and infant health outcomes across the state. Funded activities align with the state’s action plan and selected priority needs. The purpose of the Illinois Adolescent Health Education Program (AHEP) is to support organizations that provide education and trainings to medical providers and other health professionals on empowering adolescents to adopt healthy behaviors and improving the overall health of adolescents. The target population for AHEP applicants must be medical providers and other healthcare professionals that (1) serve adolescents, ages 11-21, that reside outside of the City of Chicago; (2) are able to develop and implement education/trainings for medical and healthcare providers that increases overall health knowledge of adolescents in Illinois; and (3) can provide data demonstrating the increase in provider and adolescent health knowledge.

Objective
The application should include: 1. A detailed Grant Project Proposal (Scope of Work) that demonstrates the need for the program within the community(ies) the organization serves. The proposal will describe what the organization requires to complete the work. If an applicant was a previous grantee of the Adolescent Health Program (AHP FY19-FY24) and provided similar services as proposed for this application, the applicant must indicate what they need to continue their current work as well as build in additional activities that will enhance their prior work. 2. A plan that outlines the development and distribution of educational materials, educational sessions/webinars, and trainings to adolescent serving medical providers and other health professionals in target communities. 3. The developed plan must address adolescent mental health needs, referral systems and other supports/resources for the target population. 4. A plan to develop new or expand existing partnerships with key stakeholders such as other Title V agencies, Local Health Departments, Federally Qualified Health Centers, community-based organizations, and faith-based organizations to improve adolescent health and well-being. Partnerships and collaborations should demonstrate newly formed relationships to further enhance the system of care for adolescent health. Applicants will be funded based on their ability to address the following program objectives within their Grant Project Proposal (Scope of Work): • Provide education and training to Illinois providers and healthcare professionals around the importance of increased adolescent health knowledge, focusing on topics including but not limited to mental health, adverse childhood experiences (ACEs), emotional well-being, tobacco and substance use, violence and injury prevention, sexual health and healthy relationships, importance of adult mentors, medical homes, etc. • Participate in Illinois DocAssist (IDA) training to learn about this service (e.g., what it is, how to use the program). • Develop and implement training on IDA services to educate and increase awareness (and use of IDA services) to Illinois medical/healthcare professionals across the state. • Develop (or update) and disseminate community resource guide focusing on mental health and social determinants of health resources within the target community.

Applicant Eligibility
Applicants must seek to educate and train medical providers/other healthcare professionals whose primary client population consist of adolescents ages 11-21, outside of the city of Chicago. Eligible applicants are limited to those agencies that are Illinois based, local health departments (LHD), federally qualified health centers (FQHC), and rural health centers (RHC) Community Based Organizations, and Community groups/centers with 501-C3 status. This grant program is competitive, and funding is limited. Awards will be based on scores and available funding. It is important to note that all applicants may not receive an award.

Preapplication Coordination
All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Criteria Selecting Proposals

Strategies must be Specific, Measurable, Achievable, Realistic, Timely, Inclusive and Equitable (SMARTIE). • Specific – Make your goals specific and narrow for more effective planning. • Measurable – Define what evidence will prove you’re making progress and reevaluate when necessary • Achievable – Make sure you can reasonably accomplish your goal within a certain timeframe. • Realistic – Your goals should align with your values and long-term objectives. • Timely – Set a realistic end date for task prioritization and motivation. • Inclusive – It includes those most impacted—into processes, activities, and decision/policymaking in a way that shares power. • Equitable – It includes an element of fairness or justice that seeks to address systemic injustice, inequity, or oppression. Grant will be awarded upon acceptance of the Notice of State award by signing the Uniform Grant Agreement by the grantee and the Director of the Illinois Department of Public Health

Range of Approval or Disapproval Time
Upon completion of the submission and review process, each successful grantee will receive a grant agreement to be signed by the entity’s authorized official. The grant agreement is not binding on the parties until it has been fully executed by the Illinois Department of Public Health. Application approval time is between 15-30 days.
Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b. Appeals must be received within 14 calendar days after the date that the grant award notice was published. Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request. The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required

Uses and Restrictions
Title V of the Social Security Act allows for 10% administrative cost. Direct Administrative Costs are capped at 10% of your total budget . These are necessary and reasonable costs needed to administer the grant that are not related to the direct provision of services. Detailed information must be provided regarding these costs. All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program unless specifically allowed under grant program guidelines. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for repayment of those funds used for the prohibited purpose. Regardless of the source of funding (federal pass-through or State), all grant-funded expenses must be compliant with Cost Principles under Subpart E of 2CFR200 unless an exception is noted in federal or State statutes or regulations.

Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.

Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
