PROGRAM INFORMATION
Short Description
The Illinois Department of Public Health (IDPH), as the authorizing body for Syringe Service Programs, will establish awards with organizations providing harm reduction services and/or supplies for opioid and other substance use/substance use disorders as part of the approved core abatement strategy to reduce opioid and other substance use overdose risk and improve the social determinants of health throughout the state with a priority to support individuals who use drugs and are most impacted by the opioid crisis and who continue to face disproportionate overdose inequities across our State. IDPH will assist in equitably implementing essential State resources.

Objective
IDPH will establish grant agreements and/or contracts with local and/or regional harm reduction/syringe services program organizations addressing opioid overdoses and substance use and equity and racial and social justice that are in alignment with the harm reduction core abatement strategy and harm reduction supplies. The services implemented through this award will enhance the ability of the State to support and expand harm reduction resources that reduce drug use harms, and reduce the risk of overdose, especially in areas that have experienced overdose disparities.
UGA Program Terms
Grant funds administered under this CSFA (including grantees of IDPH) are required to adhere to the requirements outlined in:
• Grant Accountability and Transparency Act (GATA) 30 ILCS 708
• Illinois Administrative Code, Government Contracts, Title 44, Part 7000
• Grantmaking, Procurement, and Property Management, and federal regulations under Grants and Agreements. 2 CFR 200

To be used to expand harm reduction services and/or distribute harm reduction supplies
Reports
Upon execution of the grant agreement, reporting shall be in accordance with the requirements set forth in the Uniform Grant Agreement and related Exhibits, which include, but is not limited
to the following:
• Time Period for Required Periodic Financial Reports. Grantee shall submit financial
reports to Grantor under Paragraph 13.1 of the Uniform Grant Agreement, and reports
must be submitted no later than 15 days after the quarter ends.
• Time Period for Close-out Reports. Grantee shall submit a Close-out Report pursuant to
Paragraph 13.2 of the Uniform Grant Agreement and no later than 15 days after this
Agreement's end of the period of performance or termination.
• Time Period for Required Periodic Performance Reports. Grantee shall submit
Performance Reports to Grantor pursuant to Paragraph 14.1 of the Uni form Grant
Agreement and such reports must be submitted no later than 30 days after the quarter
ends.
• Time Period for Close-out Performance Reports. Grantee agrees to submit a Close-out
Performance Report, pursuant to Paragraph 14.2 of the Uniform Grant Agreement, and
no later than 30 days after this Agreement's end of the period of performance or
termination.
(a) Objective A. Provide funding to harm reduction and other organizations that engage low-barrier syringe service programs via notice of funding opportunity or other GATA/procurement approved methodology.
1) Deliverable: Number of providers funded for expanded service provision each year.
2) Deliverable: Organization specific funding awarded each year.
3) Deliverable: Total number of communities impacted by funding by IDPH region.
4) Deliverable: Total number of rural and disparate communities impacted by funding by DPH region.
5) Deliverable: Quarterly and total number of additional people served by expanded harm reduction services by DPH region.
6) Deliverable: Quarterly and total number of individuals connected to treatment and other supportive programs.

(b) Objective B. Provide funding to harm reduction and other organizations that engage syringe service programs to support the supplies and materials not otherwise funded/reimbursed through other means.
1) Deliverable: Quarterly and total number of individuals who receive expanded supplies from funded harm reduction services by DPH region.
2) Deliverable: Total of organizations receiving expanded harm reduction supplies.
3) Deliverable: Total by type of supplies purchased by harm reduction organizations receiving expanded harm reduction supplies funding.
