Short Description
The Healthcare Tele mentoring Expansion Project will provide funding to eligible organization(s) to develop a plan for establishment or expansion of technology-enabled tele mentoring learning programs and services (Phase I-Planning). Programs should assist in the development of strategies focused on enhancing health care outcomes for patients with a variety of conditions to improve health of Illinois residents. Based on successful completion of Phase I, and availability of State funding, the awarded applicant(s) will be awarded non-competitive continuation grants to implement the plan (Phase II-Implementation). Implementation grants will be awarded to up to two grantees for a total combined amount of $5,000,000. Healthcare Tele mentoring Expansion Project: Approved applicants must be in Illinois and shall be eligible for State funding in accordance with rules developed by the Department of Public Health. Per Illinois legislation, Health Care Tele mentoring means the following: 1. Based on interactive video or phone technology that connects groups of local health care providers or professionals in urban and rural underserved areas with specialists in regular real-time collaborative sessions; 2. That is designed around case-based learning and mentorship; and 3. Helps local health care providers or professionals gain the expertise required to more effectively provide needed services.

Sec. 2310-730. Health care tele mentoring. (a) Subject to appropriation, the Department shall designate one or more health care tele mentoring entities based on an application to be developed by the Department. Applicants shall demonstrate a record of expertise and demonstrated success in providing health care tele mentoring services. The Department may adopt rules necessary for the implementation of this Section. Funding may be provided based on the number of health care providers or professionals who are assisted by each approved health care tele mentoring entity and the hours of assistance provided to each health care provider or professional in addition to other factors as determined by the Director.

A30 ILCS 708, TITLE 44: CHAPTER I: Section 7000
Objective
1) A plan to provide tele mentoring services based on a) needs identified through review of evidence in the scientific literature; b) a targeted local needs assessment of healthcare professional groups to identify knowledge and practice barriers to be overcome and facilitating factors that can be leveraged; or c) Needs identified by IDPH. The plan shall include I) A description of the tele mentoring services to be provided including a. The conditions for which tele mentoring will be provided to improve services. b. For each condition identified through review of scientific evidence or needs assessment: I. A description of the need for tele mentoring services to improve services for the impacted patient group ii. A description of the geographic locality for tele mentoring services to be provided iii. A description of the targeted health care professionals, including type of professional and number expected to participate iv. The name, credentials, experience, and expertise of the individuals providing tele mentoring services (If known. Otherwise, describe organization’s ability to secure needed professional expertise.) v. A plan for reaching the targeted health care professionals to ensure awareness and use of tele mentoring services vi. The cadence and format(s) of tele mentoring activities designed to maximize both participation and benefit of tele mentoring, including the total number of hours of tele mentoring services provided. vii. The number of healthcare professionals to be provided tele mentoring services and the number of hours of services to be provided viii. A description of the training materials to be provided to service recipients at no cost c. A description of the organization’s approach to and capacity to meet condition-specific requests from IDPH. 2) A description of the infrastructure necessary to effectively deliver tele mentoring services, noting whether each component is currently available or needs to be developed/expanded, including, but not limited to: I) A description of the process by which participants register to access tele mentoring services, including availability of email, phone, or other support to address questions regarding the registration process ii) A description of platforms through which tele mentoring services will be delivered iii) A description of any specialized equipment necessary to effectively deliver tele mentoring services iv) A description of networks (existing or to be established) through which tele mentoring services will be marketed 3) A description of plans for Impact Assessment Reporting either through quantitative survey and/or interviews to gauge the effectiveness of the training and consultations provided that includes number of professionals trained, geographic distribution, existing health care professional practice details, changes in their practice as a result of the training. If the tele mentoring service has access to health outcomes data, then they would ideally also assess associations between the intervention and performance on selected health outcome indicators relevant to their targeted professional group or service. 4) A detailed workplan including a) an outline detailing the organization’s approach for providing tele mentoring training. b) detailed listing of the tele mentoring and associated services being proposed to meet the targeted audience. c) plan and methodology for creating and implementing the tele mentoring training program. d) Any other resources being used by your organization including any partnerships proposed for the project e) Agreement to track and report on required Performance Indicators as required by the IDPH grants management program 5) A detailed budget by line item and justification which should additionally include: a) An outline of the personnel supported by the grant. Include name and title (If known. If not known, note “TBD”) of all staff participating on the project, percent of time projected that will be reimbursed through the grant funding,

The grantee must be an organization in the state of Illinois. The successful applicant must demonstrate that they understand the healthcare tele mentoring expansion needs in Illinois and can effectively provide planning, logistical operations, communications/technology, marketing, and project management staffing, tools, and resources necessary for project success. The applicant must have experience with establishing healthcare tele mentoring programs and be knowledgeable about the non-governmental landscape across the state of Illinois that can perform this service. The applicant must provide evidence that it has implemented successful projects that are similar in scope.

Preapplication Coordination
All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification are required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Criteria Selecting Proposals
Applications will be reviewed and graded based on a 100-point scoring rubric. The categories are as follows: d. Understanding Need and Scope of Work (10 points)-Describe the need for expansion of tele mentoring in Illinois and understanding of the scope of work expected to be supported by funding through June 30, 2026. e. Organizational Capacity (25 points)-Describe organizational capacity and relevant experience, including conditions addressed, access to expertise, ability to reach target audience, description of available platforms, and Project Director name, bio, and curriculum vitae. f. Project Planning Approach (20 points)-Project planning approach including leveraging of existing efforts, tools, resources and networks and how conditions, communities or geographic regions to be addressed will be selected. g. Budget (5 points)-Budget is reasonable and justified in budget narrative. h. Project Management (5 points)-Approach to project management; performance monitoring; progress and financial reporting. I. Award Procedures
Grant will be awarded upon acceptance of the Notice of State award by signing the Uniform Grant Agreement by the grantee and the Director of the Illinois Department of Public Health.

Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b. Appeals must be received within 14 calendar days after the date that the grant award notice was published. Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request. The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required.

Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.

Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
