Short Description
Agency Specific Content for the Notice of Funding Opportunity In Illinois there are twenty Continuum of Care organizations developed by the U.S. Department of Housing and Urban Development (HUD) to provide a network of homeless services to serve the entire state. Funding for the Homeless Prevention Program is allocated to each of these organizations utilizing HUD's homeless distribution formula. The Continuum of Care organizations in turn solicit proposals from local providers that serve homeless families and individuals. Each Continuum of Care organization then recommends agencies to receive Homeless Prevention Program funding from IDHS. Upon execution of a contract, the Homeless Prevention Program providers receive payments and implement the program. Agencies applying for Homeless Prevention Program funding are required to be a member of a State of Illinois Continuum of Care organization. Scope of Services Homeless Prevention Providers must provide one or more of the following services: • rental/mortgage assistance; • rental/mortgage arrears; • utility assistance and/or arrears; • rent/security deposit; The total dollar amount of the above items must be equal to or lesser than six months' worth of the client's rent or mortgage amount. • case management, which is the coordination of acquisition, delivery and use of program services, not to exceed 15% of the total grant; • supportive services directly related to the prevention of homelessness or repeated episodes of homelessness. Supportive services include, but are not limited to: advocacy, alcohol abuse, child care, counseling, education, employment, English as a Second Language, follow up, health, dental, HIV and Aids Related, housing location/inspection, legal/referral, outreach, mental health, substance abuse, and transportation services.

Appeals
In accordance with GATA Administrative Rules, Section 350, Merit Based Review of Grant Applications, a merit-based application review is required for competitive Grants and Cooperative Agreements, unless prohibited by State or Federal statute. The appeals process is set forth in section 7000.350 available here: http://www.ilga.gov/commission/jcar/admincode/044/044070000D03500R.html A. Competitive grant appeals are limited to the evaluation process. Evaluation scores may not be protested. Only the evaluation process is subject to appeal and shall be reviewed by IDHS' Appeal Review Officer (ARO). B. Submission of Appeal An appeal must be submitted in writing to Angela Campo at Angela.Campo@illinois.gov who will send to the ARO for consideration. An appeal must be received within 14 calendar days after the date that the grant award notice has been published. The written appeal shall include at a minimum the following: 1. DHS will respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required. c. The appealing party must supply any additional information requested by DHS within the time period set in the request. D. Resolution a. The ARO shall make a recommendation the Agency Head or designee as expeditiously as possible after receiving all relevant, requested information. b. In determining the appropriate recommendation, the ARO shall consider the integrity of the competitive grant process and the impact of the recommendation on the State Agency c. The Agency will resolve the appeal by means of written determination. d. The determination shall include, but not be limited to: i. review of the appeal ii. appeal determination iii. rationale for the determination

Reports
A. Time Period for Required Periodic Financial Reports. Unless a different reporting requirement is specified in Exhibit G, Grantee shall submit financial reports to Grantor pursuant to Paragraph 13.1 and reports must be submitted no later than 10 days after the quarter ends. B. Time Period for Close-out Reports. Grantee shall submit a Close-out Report pursuant to Paragraph 13.2 and no later than 45 days after this Agreement's end of the period of performance or termination. C. Time Period for Required Periodic Performance Reports. Unless a different reporting requirement is specified in Exhibit G, Grantee shall submit Performance Reports to Grantor pursuant to Paragraph 14.1 and such reports must be submitted no later than 10 days after the quarter ends. D. Time Period for Close-out Performance Reports. Grantee agrees to submit a Close-out Performance Report, pursuant to Paragraph 14.2 and no later than 45 days after this Agreement's end of the period of performance or termination. Grantee shall submit quarterly Periodic Performance Report (GOMBGATU-4001 (N-08-17)). Grantee shall submit a quarterly Periodic Financial Report (GOMBGATU-4002 (N-08-17)). Reported expenses should be consistent with the approved annual grant budget. Any expenditure variances require prior Grantor approval in accordance with Article VI of the Uniform Grant Agreement to be reimbursable.
Audits
Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor's Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.334, unless a different retention period is specified in 2 CFR 200.334 or 44 Ill. Admin. Code 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the records must be retained until all litigation, claims or audit exceptions involving the records have been resolved and final action taken.
