Short Description
The Local Health Protection Grant (LHPG) provides funding to certified local health departments to ensure that basic levels of protection for Illinois residents are maintained at the community level for infectious diseases, food protection, safety of the potable water supply, and private sewage disposal. In addition, for a limited number of pre-identified certified local health departments, Pre-exposure Prophylaxis (PrEP) funding is available to assist in establishing PrEP site initiatives in local health department sexually transmitted disease (STD) clinics by building infrastructure to enroll clients in PrEP to decrease the number of new cases of HIV infection by increasing access to PrEP.

Objective
The Local Health Protection Grant (LHPG) provides funding to certified local health departments to ensure that basic levels of protection for Illinois residents are maintained at the community level for infectious diseases, food protection, safety of the potable water supply, and private sewage disposal. The goal of the PrEP funding is to decrease the number of new cases of HIV infection by increasing access to PrEP by establishing PrEP site iniatives in local health department STD clinics and other designated sites by building infrastructure to enroll clients in PrEP. During the cours of the grant period, the grantee must show progression toward implementation of a PrEP program following the Illinois Department of Public Health, HIV/STD Sections’ PrEP Guidance.

What would a new grantee need to know regarding what is acceptable or not acceptable under this program to help them be successful?
The Local Health Protection Grant (LHPG) program provides funding to all certified local health departments to ensure that basic levels of protection for Illinois residents are maintained at the community level for infectious diseases, food protection, safety of the potable water supply, and private sewage disposal.
Each certified local health department must implement the following health protection programs: a comprehensive program to control infectious diseases; a comprehensive food protection program; a program to assure provision of safe, potable supplies of water for drinking, culinary, and sanitary purposes, with the focus on non-community, semi-private, and private water supplies; and a program to prevent the transmission of disease organisms, environmental contamination, and nuisances resulting from improper handling, storage, transportation, and disposal of sewage from private sewage disposal systems.
Each certified local health department must administer these health protection programs in accordance with the Local Health Protection Grant Code (77 Ill. Adm. Code 615).
Additional funding for Pre-exposure Prophylaxis (PrEP) is available for a limited number of pre-identified certified local health departments to help establish PrEP site initiatives in local health department sexually transmitted disease (STD) clinics by building infrastructure to enroll clients and decrease the number of new HIV infection cases.
Only certified local health departments in Illinois are eligible to apply for grant funding.
Cost sharing or matching funds are not required; however, applicants may voluntarily list cash or in-kind amounts and sources in their grant budgets.
Indirect costs are not allowed. Section 615.210 (77 Ill. Adm. Code 615) states that LHPG funds are intended to supplement other Federal, State, and local funds available to support local health protection programs. LHPG funds are therefore not intended to cover all costs associated with provision of health protection programs.
Grant funds may be used by certified local health departments for any health protection program or service including, but not limited to, Infectious Diseases, Food Protection, Potable Water Supply, and Private Sewage Disposal. Grant funds are intended to supplement other Federal, State, and local funds available to support local health protection programs, including the four programs that must be assured. Provided the four programs are assured, the local health departments may use the LHPG funds for any health protection program, activity or service, or for shared management or administrative support costs.
PrEP program funding must be used for expenses related to the planning for and implementation of PrEP programs.
The LHPG program is a non-competitive grant. All eligible applicants will receive a grant award. There are no merit-based review criteria for evaluating the applications. Each application will be reviewed for completeness and accuracy. Applications lacking any information required will be returned for correction and re-submission.
Upon execution of a grant agreement, the Department shall authorize a lump-sum payment in the amount of one hundred percent (100%) of the total Award. Payments to the grantee are subject to the Grantee's submission and reconciliation of eligible costs through quarterly reports to the Department.
All applications must be submitted through EGrAMS. All sections of the application must be completed, and the application must be validated, certified, and submitted through EGrAMS.
The Department provides oversight and funding to Grantees; monitors activities and performance to ensure compliance with the terms of the Program, as described in Grantee applications and agreements; and provides technical expertise and consultation by the Office of Health Protection and other designated staff within the Department.
The Department conducts program reviews every three years in accordance with 77 Ill. Adm. Code 615.220 and the schedule developed by the Office of Health Protection.
What program-specific rules, terms, or conditions do our grantees need to know about to appropriately implement this program?
Applicants must list at least one objective for each of the four LHPG program areas (Infectious Diseases, Food Protection, Potable Water, and Private Sewage Disposal). If any of the four LHPG program areas does not apply, applicants must include a brief explanation.
Each objective must identify at least one activity for LHPG and two activities for PrEP program funding. For each activity, list the responsible staff, the date or date range for the activity, and the expected outcome (if applicable).
Grantees shall submit quarterly performance reports and quarterly expenditure reports, as well as an annual self-evaluation for the Food Protection Program. Quarterly performance reports and the annual self-evaluation must be submitted through an online application on the IDPH Web Portal. Quarterly expenditure reports may be e-mailed (as a scanned PDF file) or mailed.
Details regarding the form of the reports, the Web Portal application, and the due dates for the quarterly reports will be included in the grant instructions packet, which will be sent to each applicant with the copy of the executed grant agreement.
Grantees shall submit quarterly expenditure reports, which reconcile eligible costs to the budget included in the grant application, within thirty (30) calendar days following the end of the quarter.
Quarterly performance reports will include the following information: a comparison of actual accomplishments to the objectives of the award established for the period; progress of the program and project as of the close of the period being reported on; a description of the use and expenditure of grant funds awarded; program statistical information for the Food Protection Program, the Potable Water Program, and the Private Sewage Disposal Program, in the format specified by the Department; any other information required by the grant instructions. Data shall be reported through the Illinois Department of Public Health Survey System, which is an online application on the Department's Web Portal. Unless otherwise specified, the Grantee’s first performance report shall cover the first three months after the Award begins.
Grantees shall submit a final close-out performance report within sixty (60) calendar days following the end of the period of performance. If this Agreement is terminated early, regardless of the reason, Grantee shall submit a final closeout performance report within sixty (60) calendar days following the effective date of termination.
Failure to submit required quarterly performance and expenditure reports may cause a delay or suspension of funding.
Grantees will provide services and agree to act in compliance with all State and Federal statutes and administrative rules applicable to the provision of services stated in the Grant Agreement. Grant applications submitted by Grantees are incorporated and made a part of their Grant Agreement.
Grantees shall ensure the provision of health protection programs including, but not limited to, infectious diseases, food protection, potable water supply, and private sewage disposal, within the Grantee's jurisdiction, in accordance with the Local Health Protection Grant Code (77 Ill. Adm. Code 615); conduct a comprehensive infectious diseases control program in accordance with 77 Ill. Adm. Code 615.300; conduct a comprehensive food protection program in accordance with 77 Ill. Adm. Code 615.310; conduct an effective potable water supply program in accordance with 77 Ill. Adm. Code 615.320; and conduct an effective private sewage disposal program in accordance with 77 Ill. Adm. Code 615.330.
Grantees will report communicable disease cases to the Department using the Illinois-National Electronic Disease Surveillance System (I-NEDSS) web-based system according to the time frames specified in the Control of Communicable Diseases Code (77 Ill. Adm. Code 690).
Grantees will maintain and publish, on its public website, in the Comprehensive Emergency Management Program (CEMP), and in other appropriate locations, 24-hour emergency contact information that the Department, hospitals, or members of the general public can contact to promptly reach a staff person to report a suspected or actual public health incident or event, in accordance with 77 Ill. Adm. Code 615.340 (b). The Grantee shall ensure that its Administrator, its environmental health staff, and its communicable diseases staff maintain their current 24-hour contact information in CEMP and in the Department's State of Illinois Rapid Electronic Notification (SIREN) system.
Grantees will maintain a current, all hazard emergency response/disaster plan for its jurisdiction, in accordance with 77 Ill. Adm. Code 615.340 (c). "All hazard" includes, but is not limited to, natural, technological, and intentionally caused emergency events, including disease outbreaks, bioterrorism, floods, severe weather, environmental and food protection incidents, and others. The emergency response/disaster plan will provide a framework for response operations of the Grantee and will outline specific actions for local response and recovery activities.
Grantees will not use the services of a subcontractor, excluding Operational Utilities, unless approved by the Department. The Department reserves the right to review all subcontracts and sub-grants at any time during the term of the Agreement.
Grantees shall assume responsibility for distribution of Grant Funds to sub-grantees for the provision of services under this Agreement and in accordance with the goals, objectives, and activities; and budget on file with, and approved by, the Department.
Grantees shall execute sub-grant agreements for services. Signed copies of all sub-grant agreements shall be submitted to the Department in the corresponding required progress report. Each sub-grant agreement shall identify the subgrantee and include a scope of services, budget period, detailed budget, and the subgrantee’s current mailing address. The Department will not pay any reimbursement to the Grantee related to sub-grantee activities until the Department has received a copy of the signed sub-grant agreement.
Grantees shall assure that all services provided by sub-grantees under established subgrant agreements are provided and documented in a timely manner and in accordance with Department policy. The Grantee shall promptly investigate any sub-grantee not performing in accordance with the sub-grant agreement. The Grantee is responsible for monitoring, investigating, and taking any needed action related to the sub-grantee to protect the integrity of the provision of services under this Agreement. Failure of the Grantee to do so may result in the rejection of claims for payment or in payments being reduced by the total amount of the value of the sub-grantee contract, until any and all requirements of this Agreement are fulfilled.
Grantees will not commingle funds between separate grants or sub-grants, even if the grants or sub-grants are related, or the same population is being served.
Grantees shall perform in accordance with the standards set forth herein, which are the minimum
thresholds of acceptable performance. Failure to meet these thresholds may result in remedial action including, but not limited to, corrective action, imposition of specific condition, and suspension or termination of the Agreement.
All performance standards for the LHPG program are contained in the Local Health Protection
Grant Code (Code) (77 Ill. Adm. Code 615). Performance standards for the infectious diseases control program are contained in Section 300 of the Code (77 Ill. Adm. Code 615.300). Performance standards for the food protection program are contained in Section 310 of the
Code (77 Ill. Adm. Code 615.310). Performance standards for the potable water supply program are contained in Section 320 of the Code (77 Ill. Adm. Code 615.320). Performance standards for the private sewage disposal program are contained in Section 330 of the Code (77 Ill. Adm. Code 615.320).
The criteria by which the Department will determine whether the Grantee is in substantial compliance with the performance standards are contained in Section 220 of the Code (77 Ill. Adm. Code 615.220).

Preapplication Coordination
Grantee Pre-Qualification - All entities must be qualified to do business with the State of Illinois. To be qualified for a grant award, an entity must: • Have a valid DUNS number; • Have a current SAM.gov account; • Not be on the Federal Excluded Parties List; • Be in Good Standing with the Illinois Secretary of State, as applicable; • Not be on the Illinois Stop Payment list; • Not on the Dept. of Healthcare and Family Services Provider Sanctions list. Entities must complete an Internal Controls Questionnaire (ICQ) as the Fiscal and Administrative Risk Assessment. The ICQ is completed once, annually. All state agencies will utilize the results of the ICQ. The entity can access the ICQ from the grantee portal. Application Procedure - All applications must be submitted via the Illinois Department of Public Health’s Electronic Grants Administration and Management System, EGrAMS.
