Short Description
This Tanning Inspection Program grant makes possible the inspection of tanning facilities in Illinois. Initial and annual inspections are conducted as required by the Tanning Facility Permit Act by Department trained certified local health department inspectors. This allows for the income received from the permit fees to be shared with the local health departments that participate in the program.

Objective
To assist the Department in the administration and enforcement of the Tanning Facility Permit Act. The purpose of the Act is to provide for a safe and sanitary environment for the public to use sunlamp products and ultraviolet lamps to induce the tanning of their skin.

Preapplication Coordination
All grant applications are submitted through IDPH's Electronic Grants Administration and Management System (EGrAMS); IDPH program staff reviews each application and makes the award through EGrAMS and award amounts are an estimate of the amount that will be paid out annually based on the fee-for-service method with inspections paying out at $100.00 to $150.00 as determined by type of inspection; all certified local health departments applications will be recommended for funding Grants are good for three years afterwhich grantees must submit a new grant application
Application Procedures
All applications must be submitted via the Illinois Department of Public Health’s Electronic Grants Administration and Management System (EGrAMS). All sections of the application must be complete
Criteria Selecting Proposals
This is a non-competitive grant. All eligible applicants will receive a grant award. There are no merit-based review criteria for evaluating the applications. Each application will be reviewed for completeness and accuracy. Applications lacking any required information will be returned to the applicant for correction and re-submission. Because the LHPG is a non-competitive, formula-based grant program, all eligible applicants will receive a grant award.
Award Procedures
Upon final approval by IDPH of the EGrAMS grant application, the applicant will be sent a grant agreement through EGrAMS. Each applicant will be required to print the grant agreement, sign it and return it to IDPH for execution. After execution, a copy of the executed grant agreement will be provided to the applicant.

Formula Matching Requirements
Award amounts are estimated based on the grantee's previous year's total reimbursement request submissions. The amount is an estimate as it may go up or down based on the opening or closing of tanning facilities in the grantee's jurisdiction. The grantee is paid only for work completed; Quarterly reports showing work completed as well as the report forms from the actual inspections to verify the work for payment request. Copies are maintained for 5 years unless in the middle of legal action or audit and they are held until completed.

Grantees must submit quarterly reimbursement requests and quarterly programmatic (facility inpsection ) reports. Grantees must maintain facility files including at minimum permit applications, copies of permits, inspection reports, complaint reports, investigational findings, correspondance to facilities and enforcement records. The files must be maintained for the three years from the final date of payment under the grant. "
