Objective
(1) Conduct ongoing surveillance and control activities for mosquitoes, ticks, rodents and other potential vectors of human disease, and conduct surveillance of animals that provide a reservoir for disease producing organisms (2) Respond to inquiries, investigate complaints, conduct evaluations, and provide technical consultation to help reduce or eliminate public health hazards and nuisance conditions associated with vectors of human disease (3) Conduct training activities for municipal governments to promote integrated pest management of human disease vectors.

Grant is only available to certified local health departments that choose to conduct vector control activities. The grant provides funding to conduct environmental surveillance, public information, human case investigation and prevention of mosquito-borne diseases such as West Nile viral encephalitis, Zika virus and other vector borne diseases, as described in the Vector Control Act (410 ILCS 95). Grantees specifically collect and ship dead birds for WNV testing; conduct inspections of mosquito production sites; work with local municipal governments to reduce the risk of WNV cases through the preventive treatment of mosquito production sites; investigate reports of mosquito production sites; conduct mosquito surveillance and collection of mosquito samples for WNV testing; and conduct public education on vector control.

Preapplication Coordination
All entities must be qualified to do business with the State of Illinois. To be qualified for a grant award, an entity must: • Have a valid DUNS number • Have a current SAM.gov account • Not be on the Federal Excluded Parties List • Be in Good Standing with the Illinois Secretary of State, as applicable • Not be on the Illinois Stop Payment list • Not be on the Department of Healthcare and Family Services Provider Sanctions list Entities must complete an Internal Controls Questionnaire (ICQ) as the Fiscal and Administrative Risk Assessment. The ICQ is completed once annually. All state agencies will utilize the results of the ICQ. The entity can access the ICQ from the grantee portal.

All applications must be submitted via the Illinois Department of Public Health’s Electronic Grants Administration and Management System (EGrAMS). All sections of the application must be complete
Criteria Selecting Proposals
This is a non-competitive grant. All eligible applicants will receive a grant award. There are no merit-based review criteria for evaluating the applications. Each application will be reviewed for completeness and accuracy. Applications lacking any required information will be returned to the applicant for correction and re-submission. Because the LHPG is a non-competitive, formula-based grant program, all eligible applicants will receive a grant award.
Award Procedures
Upon final approval by IDPH of the EGrAMS grant application, the applicant will be sent a grant agreement through EGrAMS. Each applicant will be required to print the grant agreement, sign it and return it to IDPH for execution. After execution, a copy of the executed grant agreement will be provided to the applicant.

Formula Matching Requirements
Grantees must submit quarterly expenditure reports and quarterly programmatic reports. IDPH reviews surveillance data submittted by grantee to ensure surveillance activites. Grantees must maintain documentation of activities conducted for a minimum of 5 years
Uses and Restrictions
Because of West Nile virus’s ability to “amplify” (multiply very rapidly in birds and Culex mosquitoes), West Nile virus (WNV) is believed to be the vector-borne disease with the greatest potential impact on Illinois. At least 80% of the grant funding must be used to enhance a mosquito vector surveillance and control program that includes testing of Culex mosquitoes and dead crows, blue jays and other birds for West Nile virus and control of larval mosquitoes of the genus Culex (the primary vectors of WNV) or other vector mosquitoes. No more than 20% of the grant funding may be used to conduct surveillance and prevention of non-mosquito vectors of human disease such as ticks, rodents, nuisance birds and other vectors of human diseases. Surveillance and control of non-mosquito vectors of human diseases may include epidemiological investigations, public information work and other appropriate disease-prevention activities, but not for more than 20% of the grant funding

All mosquito control insecticides purchased and/or used by the grantees or cooperating agencies must be registered with the IL Dept of Agriculture. Personnel applying those insecticides must be appropriately licensed by the Illinois Dept of Agriculture. Contact the IL Dept of Agriculture at 217-785-2427 for more information. If required, LHDs or the agencies cooperating with the grantee must have a NPDES [National Pollution Discharge Elimination System] permit from the IL Environmental Protection Agency - call 217-782- 6038 for more information.
