Short Description
The Quality of Life Grant will support HIV prevention and treatment engagement services for people living with HIV Disease by making grants to public or private entities in Illinois that serve people living with HIV disease and/or the highest at-risk populations for acquiring HIV infection.

Objective
The Quality of Life (QOL) Endowment Fund was created as a special fund in the Illinois State Treasury. The net revenue from the Quality-of-Life special instant scratch-off game is deposited into the Fund for appropriation by the Illinois General Assembly solely to the Illinois Department of Public Health (IDPH) “for the purpose of HIV/AIDS-prevention education and for making grants to public or private entities in Illinois for the purpose of funding organizations that serve the highest at-risk categories for contracting HIV or developing AIDS.” QOL Grants are targeted to serve at-risk populations in proportion to the distribution of recent reported HIV Disease cases in Illinois, stratified by risk, race and ethnicity as reported to IDPH HIV Surveillance. To encourage a diverse set of applications collectively requesting to serve all the prioritized populations, additional points will be awarded to applicant organizations proposing to serve multiple prioritized populations and for requesting to serve populations under-requested in the last funding cycle

Applicant Eligibility
To be eligible for grant awards, recipient organizations must demonstrate engagement and accomplishment in HIV prevention/education, HIV care and/or supportive services for people living with HIV and/or at-risk populations. Applicant organizations may be local health departments or not-for-profit private community-based organizations including volunteer or religious organizations which effectively engage prioritized risk populations including gay or bisexually active males, high risk heterosexual women and men, impacted by poverty, and current or former users of injection drugs.

Award Procedures
Successful applicants shall demonstrate the ability to execute the grant project according to project requirements, including appropriate targeted audiences and expected outcomes. Successful applicants will include a work plan inclusive of all aspects of their Scope of Work and project requirements while specifying SMART objectives. Successful applicants will include a RWPB Detail Budget Attachment in which costs are allocable, reasonable, and appropriate. Successful applicants will provide a grant project budget in EGrAMS in which costs are allocable, reasonable, and appropriate. Details and associated narratives for all grant costs are required to be included within the grant application budget framework.

The Department intends to award applicants within six weeks of completion of an agreement.
Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the GATA section of the IDPH website (www.dph.illinois.gov/GATA). o Appeals must be received within 14 calendar days after the date that the grant award notice was published. o Appeals must include the following information: ? The name and address of the appealing party ? Identification of the grant ? A statement of reasons for the appeal ?
