Short Description
This grant program helps to support existing screening programs conducted by certified local health departments and non-profit agencies. Applicant agencies must be currently operating an existing screening program which provides mandated vision and hearing screening services, per the Illinois Child Vision and Hearing Test Act (410 ILCS 205), in all licensed preschool/daycare facilities within the designated area(s) for children 3 years of age and older who are not yet in a regular school program. Mandated vision and hearing screening services will be provided in accordance with requirements found in the Illinois Child Vision and Hearing Test Act (410 ILCS 205) and the Illinois Administrative Code, Title 77, Part 675 Hearing Screening and Part 685 Vision Screening. All staff conducting vision and hearing screening must be certified as vision and hearing screening technicians, through the Department's training program. Objective
Provide mandated vision and hearing screening services for all 3 - 5 year olds enrolled in licensed preschools and day care centers within the designated area. Screeners are trained and certified by the IDPH and follow requirements found in the IL Administrative Code for vision and hearing screening. Grantees must submit a monthly Vision and Hearing screening Program Report for reimbursable vision and hearing screening services at the reimbursement rate specified.

Applicant Eligibility
Eligible agency types include: (1) Tax Exempt; (2) Governmental; (3) Educational. Applicant agencies must be currently operating an established screening program which provides mandated vision and hearing screening services in accordance with requirements found in the Illinois Child Vision and Hearing Test Act (410 ILCS 205), in all licensed preschool/daycare facilities within the designated area(s) for children 3 years of age and older who are not yet in a regular school program. Applicant agencies must have documentation of existing Program screening activity on file with the IL Department of Public Health in the form of current Annual Hearing and Vision Conservation Reports. They must also document properly credentialed staffing sufficient to support the required screening activity in the designated area.

Criteria Selecting Proposals
Applications will be reviewed and approved for funding based on the ability to serve the target population and satisfactory prior year performance. Documentation of existing screening program activity must be on file with the Il Department of Public Health in the form of current Annual Hearing and Vision Conservation Reports. Documentation of properly credentialed staff sufficient to support required program goals must also be provided. Review is to be set up as Individual Open Call Review.

Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b. Appeals must be received within 14 calendar days after the date that the grant award notice was published. Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request. The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required..

Grantee agencies must be currently operating an existing screening program which provides mandated vision and hearing screening services, per the Illinois Child Vision and Hearing Test Act (410 ILCS 205), in all licensed preschool/daycare facilities within the designated area(s) for children 3 years of age and older who are not yet in a regular school program. Grantee agencies may request reimbursement of up to $7.00 per screening test provided to the target population, up to the total grant award amount. All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program unless specifically allowed under grant program guidelines. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for repayment of those funds used for the prohibited purpose. Regardless of the source of funding (federal pass-through or State), all grant-funded expenses must be compliant with Cost Principles under Subpart E of 2CFR200 unless an exception is noted in federal or State statutes or regulations. Allowability Allowable – All grant funds must be used for items that are necessary and reasonable for the proper and efficient performance of the grant and may only be used for the purposes stated in the grant agreement, work plan, and budget. Items must comply with all applicable state and federal regulations. Allocable – Grant-funded costs must be chargeable or assignable to the grant in accordance with relative benefits received. The allocation methodology should be documented and should be consistent across funding sources for similar costs. Reasonable – The amounts charged for any item must be reasonable. That means the nature and amount of the expense does not exceed what a prudent person under the same circumstances would expend; and that the items are generally recognized as ordinary and necessary for the performance of the grant.
Reports
Grantees must submit a monthly Vision and Hearing Screening Program Report for reimbursable vision and hearing screening services at the reimbursement rate specified (up to $7.00/test). A report must be submitted for each month of the grant term documenting screening activity for the month. If no screenings were provided during the month, a report must be filed with the Department indicating no screenings were provided. Vision and Hearing Annual Conservation Reports must be filed by June 30 each year.
Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
