Short Description
The purpose of the Illinois Violent Death Reporting System (IVDRS) is to develop a state-wide data repository related to violent deaths. With these data, policy makers and researchers will be able to analyze the causes and correlations of violent deaths in order to develop effective prevention policies and programs for the State of Illinois. This is a direct appropriation for Northwestern University. The grant will provide $76,700 to maintain and expand the IVDRS between July 1, 2023 and June 30, 2024. Applicant must submit a project plan that describes how the award will be executed. The project plan should include necessary detail to enable the agency to manage the grant agreement activity against planned project performance.

Objective
This grant award will be made to provide Northwestern University funds to collect and enter violent death data into the online Illinois Violent Death Reporting System (IVDRS) in multiple counties, in addition to checking the data for accuracy, representativeness and timeliness. Activities also include promoting the use of IVDRS data, engaging in data dissemination efforts and convening an advisory committee.

Application Procedures
Applications must be submitted via the Illinois Department of Public Health's Electronic Grants Administration and Management System (EGrAMS), accessible at idphgrants.com. Applications will be reviewed and scored on the following criteria outlined in the Criteria section by the Illinois Department of Public Health, Violence and Injury Prevention Section.
Criteria Selecting Proposals
Applications will be reviewed by Program staff, and if criteria are met, shall be considered successful. The grantee must work closely with the IDPH Office of Health Promotion (OHPm), Division of Emerging Health Issues’ Violence and Injury Prevention Section leadership, attest to meet grant deliverables, provide adequate budget and justification for allocation of project resources.
Award Procedures
The grant is non-competitive. Application will be reviewed by the Illinois Department of Public Health, Division of Chronic Disease, Injury and Violence Prevention Program staff and approval will be completed based on prior grant guidelines, timely submission of reports and fiscal monitoring.

Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b. Appeals must be received within 14 calendar days after the date that the grant award notice was published. Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request. The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required..

Uses and Restrictions
All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for recouping of those funds used for the prohibited purpose. Expenditure reports must be submitted quarterly. To be reimbursable under the Department/Office of Health Promotion Grant Agreement, expenditures must meet the general criteria outlined in the NCFI.

Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
