PROGRAM INFORMATION
Short Description
The purpose of the Illinois Violent Death Reporting System (IVDRS) is to develop a state-wide data repository related to violent deaths. With these data, policy makers and researchers will be able to analyze the causes and correlations of violent deaths in order to develop effective prevention policies and programs for the State of Illinois. This is a direct appropriation for Northwestern University. The grant will provide $76,700 to maintain and expand the IVDRS between July 1, 2024 and June 30, 2025. Applicant must submit a project plan that describes how the award will be executed. The project plan should include necessary detail to enable the agency to manage the grant agreement activity against planned project performance

Preapplication Coordination
All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State. Criteria Selecting Proposals
Applications will be reviewed by Program staff, and if criteria are met, shall be considered successful. The grantee must work closely with the IDPH Office of Health Promotion (OHPm), Division of Emerging Health Issues’ Violence and Injury Prevention Section leadership, attest to meet grant deliverables, provide adequate budget and justification for allocation of project resources.
Award Procedures
Applications will be reviewed for content, work plan activities, budget proposals, and required application supplemental material. In addition, based on adherence to prior grant guidelines, timely submission of reports and fiscal monitoring. The Health Equity Checklist is a scored portion of the IDPH grant application. In public health, health equity is the opportunity for everyone to reach their full health potential, regardless of any social determined circumstance. There are seven questions worth 35 points.
Grant will be awarded upon acceptance of the Notice of State award by signing the Uniform Grant Agreement by the grantee and the Director of the Illinois Department of Public Health
Deadlines
Applications must be submitted through the Illinois Department of Public Health Electronic Grants Administration & Management System (EGrAMS) accessible at idphgrants.com. Applications must be received by the close of business (5:00 p.m.) on August 30, 2024. The anticipated announcement will be no later than 4 weeks after the submission of the application.
Uses and Restrictions
All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for recouping of those funds used for the prohibited purpose. Expenditure reports must be submitted quarterly. To be reimbursable under the Department/Office of Health Promotion Grant Agreement, expenditures must meet the general criteria outlined in the NCFI.
Reports
Grantees are required to submit quarterly performance reports on their work plan objectives and quarterly fiscal reporting in EGrAMS within 30 days after the reporting period. Reimbursement reports cannot be approved unless progress reports are up-to-date. Failure to submit required reports in a timely manner will result in delays with approval of reimbursements. The grantee will ensure quarterly reports are submitted.
Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
