Short Description
The Well-Integrated Screening and Evaluation for Women Across the Nation (WISEWOMAN) Program extends preventive health services to women who are participants of the National Breast and Cervical Cancer Early Detection Program (NBCCEDP).

Objective
The Illinois WISEWOMAN Program provides funds to Illinois Breast and Cervical Cancer Program providers to provide cardiovascular (CVD) screening to women, ages 35 to 64, whose income is at or below 250% of the federal poverty level, along with lifestyle programs to reduce CVD risk factors.

Grant requirements mandate that IWP collect information on the specific goals, objectives and tasks to be accomplished during each quarter of the grant year, as well as the expected outcomes and the method(s) used to measure those outcomes. Lead Agency Work Plans are being provided under separate cover. The Work Plans include the specific goals, objectives and tasks by quarter, along with the expected outcomes and a description of how the outcomes will be measured. Each Lead Agency must report on the progress made towards these tasks and expected outcomes in the quarterly progress reports: 1) progress reports each quarter of the Fiscal Year that illustrate the grantee’s success in meeting the specific measurable goals, objectives and tasks outlined in the work plans; 2) agencies will inform IDPH of staff changes as they occur within two weeks of the change; 3) agencies will provide other monthly reporting forms as required by the program.

Illinois WISEWOMAN Program grants are awarded to IWP Lead Agency grantees, which are required to also administer the Illinois Breast and Cervical Cancer Program grants, since recipients of IWP services must be an IBCCP participant.

All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active DUNS number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State. Preapplication Coordination
Each applicant (unless the applicant has an exception approved by the Federal or State awarding agency under 2 CFR § 25.110(d)) is required to: i. Be registered in SAM before submitting its application. The application format is provided via EGrAMS, which must be completed in its entirety. Criteria Selecting Proposals
Illinois WISEWOMAN Program grant applications are reviewed by IDPH staff to determine if the application is complete and accurate. The following components must be submitted by all applicants: Applicant Information, Applicant Grant History, Applicant’s Agreement to Adhere to Scope of Work, Grant Budget, Certifications, and Attachments (W-9, medical provider listing, and key staff resumes/CPR certifications). A review checklist is utilized to ensure all required components have been submitted. If no corrections or additions are needed, the applicant will receive a grant award. If corrections or additions are needed, the applicant will be contacted to request the required items. Upon submission of the required items, the applicant will receive a grant award.
Award Procedures
Upon completion of the application submission and review process, and the provision of any missing or corrected items as appropriate, each successful grantee will receive a grant agreement to be signed by the entity’s authorized official. The grant agreement is not binding on the parties until it has been fully executed by the Illinois Department of Public Health and a fully executed copy has been provided to the grantee.

Uses and Restrictions
Illinois WISEWOMAN Program grant applicants must submit budget items in accordance with the budget categories detailed in EGrAMS: Program Management, Patient Services (Clinical), and Healthy Behavior Support Options. Indirect costs, research and development, and construction costs are not allowed to be reimbursed by the Illinois WISEWOMAN Program.

Audits
Grantees are be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.334, unless a different retention period is specified in 2 CFR 200.334 or 44 Ill. Admin. Code 7000.430(a) and (b).

Program Accomplishments
1. Deliver risk reduction counseling to 100% of women screened. Risk reduction counseling includes appropriate referral to health coaching, community resources, or lifestyle programs. 2. Follow up with 100% of women with abnormal blood pressure values (greater than or equal to 120/80). 3. Ensure that 80% of women referred to a lifestyle program or health coaching participate in the program. Participation is defined as attendance at a minimum of one lifestyle program or coaching session. 4. Ensure that 60% of women who participate in a lifestyle program or health coaching complete the program. Completion is defined as the number of sessions that the evidence base for the program is determined to be required for behavior change.
