Short Description
Communities of Color Special At Risk Population: The initiative is funded through State of Illinois general revenue funding, and funds HIV prevention, education, testing, and care programs targeting communities of color - specifically programs with the capacity to reach special at risk minority populations to include, but not limited to, men who have sex with men, people experiencing homeless, ex-offenders, sex workers, those with a history of mental illness or substance abuse, and other difficult to reach populations. These specific efforts are necessary due to the unique problems that many of these individuals may face that places them at greater risk for HIV disease.

Objective
Key staff members must have a minimum of two years’ experience conducting Centers for Disease Control and Prevention HIV interventions with the proposed targeted HIV special at risk population and provide documentation of that experience with the application. Grantee organization and key staff members must demonstrate the capacity to successfully provide services to members of the proposed targeted HIV special at risk population. Grantee must have documented proof of appropriate linkage agreements, and an explanation as to how these collaborators will contribute to the overall accomplishment of project description and objectives. Key staff members must provide documentation of successful completion of training in any identified DEBI to be utilized. Grantee organization and key staff members must provide documentation of ability to provide HIV counseling and testing services if those services are a component of the application. Documentation should include proof of the completion of a Department of Public Health HIV/AIDS Section approved HIV course, or the completion of a Chicago Department of Public Health HIV/AIDS approved HIV course. Organization must demonstrate they have the fiscal/organizational capacity to administer and implement all proposed activities and tasks. Each applicant must be in good standing with the Grant Accountability & Transparency Act Unit operating under the Governor’s Office of Management & Budget - https://omb.illinois.gov. Successful applicants will have completed the GATA Pre-Qualification information and the Fiscal and Administrative Risk Assessment (Internal Controls Questionnaire – ICQ).

Criteria Selecting Proposals
Upon submission, applications will be reviewed for completeness and for consistency with program parameters. Applications will then be scored out of 100 points scored as follows: Needs 10% Capacity 10% Clearly Defined Scope of Work 10% Quality 10% SMART 10% Check List Compliance 35% Quality 15%
Award Procedures
Applicants will receive notification via Egrams of the award once all of the applications in E-grams are reviewed and scored.

Appeals
For merit based reviews, only the evaluation process is subject to appeal. Competitive grant appeals are limited to the evaluation process. Evaluation scores or award decisions may not be protested.
Renewals
The Communities of Color grant is a three -year grant competitively awarded in year 1 with the option of 2 years non-competitive renewal applications. Funding for subsequent years shall be contingent upon available funds, and applicant performance evaluation conducted by the Illinois Department of Public Health.

Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Records Retention. Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the records must be retained until all litigation, claims or audit exceptions involving the records have been resolved and final action taken.
