Reports
Per federal reporting timeline A Federal Financial Report, Standard Form SF- 425 (and Federal Financial Report Attachment, SF-425A, if required for reporting multiple awards), must be submitted annually. These reports are due 90 days after the reporting period ending December 31st. The final SF-425 (and SF-425A, if applicable) must be submitted either with the final payment request or no later than 120 days from the expiration date of the award. These forms may be found at https://www.grants.gov/web/grants/forms.html. F. PROGRAM PERFORMANCE REPORTS. The recipient shall perform all actions identified and funded in application/modification narratives within the performance period identified in award. In accordance with 2 CFR 200.301, reports must relate financial data to performance accomplishments of the federal award. The Recipient shall submit annual performance reports. These reports are due 90 days after the reporting period ending December 31st. The final performance report shall be submitted either with the Recipient’s final payment request, or separately, but not later than 120 days from the expiration date of the award. - Additional pertinent information: Please send all reports to SM.FS.R9SPFgrants@usda.gov on or before their due date to prevent delays in payment.
Audits
Audits - Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
